배경 및 목적:최근 단백질 분석법은 많은 질환의 병태생리에 대한 연구에 광범위하게 이용되고 있다. 급성 관동맥 증후군은 염증, 혈소판의 활성화, 응고 및 혈전 형성 등과 관련된 여러 인자들과 관련되어 있으나 급성 관동맥 증후군의 단백질 발현 양상에 대한 연구는 드물다. 이에 저자는 급성 관동맥 증후군 환자의 혈액을 이용해 단백질 분석을 실시하여 단백질 발현의 차이를 연구하였다.방 법:대상 환자는 급성 관동맥 증후군 24명을 대상으로 하였으며 이중 급성 심근 경색증군이 12명, 불안정 협심증군이 12명이었다. 대조군으로는 나이, 성별 및 고혈압, 당뇨의 병력이 유사한 13명을 대상으로 하였다. 대상 환자의 혈액을 이용하여 2차 전기 영동법을 이용하여 단백질을 분리하였고 분리된 단백질을 이미지 분석한 후 환자간에 차이를 보이는 단백질은 질량 분석기로 분석한 후 데이터 베이스를 이용하였다. 차이를 보이는 spot의 값은 그 spot의 %volume(해당 spot의 volume/전체 gel의 총 spot의 volume)을 가지고 비교하였고 단위는 AU로 사용하였다.
결 과:
세군간에 차이를 보이는 두 개의 지역(area 1, 2)이 있었으며, area 1은 fibrinogen gamma chain이었으며 세 개의 isoform이 있었다. 세 개의 isoform 모두 대조군에 비해서 불안정 협심증군 및 급성 심근경색증군에서 높았다. Area 2는 fibrinogen beta chain이었으며 네 개의 isoform이 있었다. 네 개의 isoform 중 세 개는 대조군에 비해 불안정 협심증군 및 급성 심근경색증군에서 높았다. 나머지 하나의 isoform은 차이가 없었다.
결 론:
급성 관동맥 증후군 환자의 혈액을 이용한 단백질 분석을 통해 fibrinogen gamma chain의 세 개의 isoforms과 fibrinogen beta chain의 네 개의 isoform을 확인하였다. Fibrinogen gamma chain의 세 개의 isoform과 fibringer beta chain의 네 개의 isoform 중 세 개가 급성 관동맥 증후군에서 가장 높았다. 급성 관동맥 증후군 환자의 단백질 발현 양상은 fibrinogen gamma와 fibrinogen beta에서 대조군에 비교해 높게 발현되었다.Background and Objectives:Proteomics is a new technology that allows the detection and identification of several proteins at a given time in a sample. There are currently few reports concerned with the proteomic study of serum from patients during acute coronary syndrome. We performed proteomics to analyze the modifications in the serum protein map of patients with acute coronary syndrome (ACS). Subjects and Methods:We investigated the serum from 12 patients who suffered with acute myocardial infarction (AMI), 12 patients with unstable angina (UA) and 13 age- and sex-matched patients as the control group. Two-dimensional electrophoresis, Coumassie staining and image analysis were performed. Mass spectrometry was performed to identify the selected spots. Results:For the two-dimensional electrophoresis with using a pH range of 3 to 10, two different areas within the serum protein map were observed, and this showed differences between the groups. In area 1, three fibrinogen gamma chain isoforms were identified. All of them were increased in the serum from the AMI and UA patients when compared with the control group. In area 2, four fibrinogen beta chain isoforms were identified. Three isoforms of them were increased in the serum from the AMI and UA patients. Conclusion:Three fibrinogen gamma chain isoforms were identified and they were increased in the serum from ACS patients. Four fibrinogen beta chain isoforms were identified and three isoforms of them were increased in the serum from ACS patients. (Korean Circulation J 2007;37:271-276)
