강현식, 한태경, 이신호, 홍혜련, 이상화, 박수현. 허리둘레와 BMI 기준에 따른 비만집단 간 아디포사이토카인 및 대사증후군 지표 비교. 운동과학, 제16권 제2호. 95-102, 2007. 본 연구의 목적은 중년여성에서 허리둘레와 체질량지수 기준에 따라 비만관련 대사증후군 지표들에 어떠한 차이가 있는지 조사하는 것이었다. 총 참여피험자는 30명의 중년여성을 대상으로 허리둘레≥80cm를 충족한 자를 비만 I 집단으로(n=18명), 허리둘레≥80cm과 체질량지수≥25kg/m2를 모두 충족한 자를 비만 II 집단으로(n=12명) 나누었다. 측정항목은 인체계측 변인(BMI, 허리둘레, 체지방, 허리-둔부비), 심폐체력, 지질(TG, TC, HDLC, LDLC), 글루코스, 인슐린, 렙틴, 아디포넥틴으로 하고 또한 당 내인성 검사를 실시하였다. 두 집단을 비교해본 결과 비만 II 집단은 비만 I 집단에서보다 체중, 체질량지수, 허리둘레, 총 복부지방, 복강내면의 지방 분포에서 유의하게 높게 나타났으나 심폐체력에선 유의한 차이가 나타나지 않았으며, 중성지방, 글루코스, 당내인성 테스트, 혈압, 렙틴도 비만 I 집단에서 보다 유의하게 높게 나타났다. 이상의 결과를 종합해 볼 때 허리둘레만을 충족시키는 집단에 비해 허리둘레와 체질량지수를 동시에 충족시키는 집단에서 대사증후군 위험인자에 대한 위험도가 상대적으로 높은 것으로 나타났으며, 이런 위험도에는 렙틴의 영향이 어느 정도 작용한 것으로 보인다. 따라서 복부비만 판단기준으로써 허리둘레와 체질량지수를 동시에 이용하는 것이 허리둘레만 혹은 체질량지수만을 이용하는 것보다 더욱더 효과적으로 대사질환 관련 위험 집단을 판단할 수 있을 것으로 사료된다.Kang, H.S., Han, T.K., Lee, S.H., Hong, H.L., Lee, S.H., & Park, S.H. The purpose of thus study was to compare group differences in adipocytokines and obesity-related metabolic syndrome indices between obesity I and obesity I groups based on the criteria of waist circumference and body mass index in mid-life women. A total of 30 women volunteers were classified as either obesity I group who met the abdominal obesity criterion only(waist circumference of ≥80 cm, n=18, 42.6±5.2yrs) or obesity II group(n=12, 45.0±4.6yrs) who met both of BMI (≥25 kg/m2) and abdominal obesity criteria. Mean values of anthropometric variables(i.e., BMI, waist circumference, BF%, WHR), cardiopulmonary fitness, circulating lipids(TG, TC, HDLC, LDLC), glucose, insulin, leptin and adiponectin were compared between the two groups. In addition, they all underwent an oral glucose tolerance test of 70 g carbohydrate for 2 hours. In the group analyses, the obesity II group had significantly higher values of obesity indices than the obesity I group, with no significant group difference in cardiorespiratory fitness. With respect to metabolic syndrome indices, the obesity II group had significantly higher values of TG, glucose, area under the curve(AUC) during oral glucose tolerance test, BP, and leptin than the obesity I group, with no group difference in adiponectin level. The current findings of the study suggest that utilizing both criteria of obesity such as BMI and waist circumference would provide additional and useful diagnostic information regarding the status of obesity-related metabolic syndrome in mid-life women.
