배경 및 목적:
허혈은 심근 조직에 가해지는 가장 흔하고 중요한 손상이다. 허혈성 전조치(ischemic preconditioning)는 허혈 상태에서도 세포의 사망을 억제하고 생존을 촉진하는 심근의 고유기능으로서, protein kinase C(PKC), 특히 ε 아형(PKCε)이 이의 신호전달 과정에서 주요 매개체로 알려져 있다.그러나 대부분의 실험에서 PKCε의 역할은 이의 억제제나 효현제 투여 후에 변화를 봄으로써 추정되었고 심근 조직에서 PKCε 발현이 허혈에 대한 보호 효과가 있는지를 직접적으로 관찰한 경우는 드물다. 본 연구는 lentiviral vector를 이용하여 PKCε를 심근세포에 발현시킴으로써 허혈 손상에 대한 보호 효과를 직접 입증하고자 하였다.
방 법:
체외 실험으로서는 신생쥐의 일차 배양을 이용하여 허혈 상태를 조성하였으며 생체 실험으로서는 성체 백서의 심근 경색 모델을 이용하였다. 배양된 심근세포는 허혈 조건을 가하기 2일전에 PKCε가 재조합된 lentiviral vector를 배양액에 첨가하였으며 심근 경색 모델에서는 관상동맥 결찰 4일전에 lentiviral vector를 심근내 주입하였다.
결 과:
결 론:
이같은 소견들은 쥐에서 PKCε의 발현이 허혈 손상으로 인한 심근세포의 사망을 억제하고 생존을 증가시킬 수 있음을 직접적으로 보여주는 증거라고 할 수 있다. 이러한 결과는 PKCε를 매개로 한 기전이 향후 허혈에 노출된 심근의 손상을 최소화하는 방편으로 이용될 수 있는 가능성을 제시하고 있다.Background and Objectives:Ischemic injury is the most common and important cause of myocardial damage. Over past decades, a number of studies have identified a protective mechanism known as ischemic preconditioning, which can block or delay cell death from ischemic injury. Protein kinase C (PKC), especially theεisoform has been proposed as a key factor in the signaling pathway of ischemic preconditioning. However, whether PKCε expression in cardiomyocytes can offer such protection from acute ischemia has not been explored. Materials and Methods:To demonstrate a direct effect of PKCε expression, a lentiviral vector system was established. Using the lentiviral vector, PKCε was introduced to neonatal rat ventricular myocytes (NRVM) cultured under ischemic conditions, and also to adult rat myocardium subject to left coronary artery ligation. Results:Compared to control, PKCε expression in cultured NRVM under ischemia resulted in preserved cell density and morphology, and a reduction in cell death (77.6±12.8% vs 58.1±7.2%, p<0.05). In adult rats, the infarcted area after coronary artery ligation was markedly reduced in myocardium injected with PKCε vector compared to control (11.4±5.3% vs 20.5±11.3%, p<0.01). Conclusion:These results provide direct evidence that PKCε is a central player in protection against cell death from acute ischemia. (Korean Circulation J 2007;37:327-333)
