본 연구에서는 장해 평가의 중요성을 알아보기 위하여, ADHD를 진단할 때 증상 기준에 해당되는 대상들 가운데 장해 기준에도 함께 해당되는 비율(진단일치율)을 알아보았다. 장해 기준은 신뢰도와 타당도가 검증된 전반적 기능 장해 평가(global measures of disability)와 특정한 증상에 대한 기능 장해 평가(measures of symptom-specific impairment) 도구를 사용하여 측정하였다. 그리고 ADHD 각 하위 유형에서 주로 나타나는 기능 장해가 무엇인지 알아보았으며 하위 유형에 따라 그 기능 장해의 비율이 다른지도 알아보았다. 그 결과, ADHD 증상 기준에 해당하는 75명(부주의 우세형 36명, 과잉행동-충동성 우세형 15명, 복합형 24명)에게 장해 기준을 적용했을 때 진단일치율이 79.2%에서 100%까지 어떠한 장해 기준을 사용했느냐에 따라 일치율이 다르게 나타났다. 그리고 ADHD 전체와 모든 하위 유형에서 사회적 기능의 장해가 가장 높게 나타났으며 하위 유형 간의 유의한 기능 장해의 차이는 없었다. 본 연구의 결과는 아동과 청소년의 ADHD 진단에 있어서 기능 장해에 대한 평가가 중요하며, 아동?청소년 ADHD의 치료에서 사회적 기능 장해에 대한 개입이 필요하다는 것을 시사하고 있다. 마지막으로 본 연구의 시사점과 제한점 및 장래연구 방향을 논의하였다.To assess the importance of functional impairment, the first aim of the present study was to investigate the diagnostic consistency between symptom and impairment criteria of Attention-Deficit/Hyperactivity Disorder(ADHD). The second aim was to compare the functional impairment of children and adolescents with ADHD according to the different measures of impairment and to ADHD subtypes. Seventy-five children and adolescents who met the full symptom criteria of the DSM-IV ADHD algorithm were assessed for impairment. In order to assess the impairment, we used the Children's Global Assessment Scale(C-GAS), a certified global measure of disability and measures, and two other symptom-specific impairment measures that were included in the Kiddie-Schedule for Affective Disorders and Schizophrenia(K-SADS) and Diagnostic Interview Schedule for child version IV(DISC-IV). As a result, diagnostic consistency was 79.2% to 100%, varying according to the impairment measure used. In our sample, impairment in social functioning was the highest and problems with parents were salient. In addition, there were no differences in impairment between ADHD subtypes. These results imply that measuring impairment is important in the diagnostic procedure of ADHD, and that it is necessary to attend to such impairment in social functioning when treating children and adolescents with ADHD. Finally, the implications and limitations of this study and suggestions for future researches have been discussed.
