본 연구는 외현적으로 유사한 행동증상과 주의력 문제를 가진 것으로 알려진 주의력결핍과잉행동장애(ADHD) 아동들과 우울장애 아동들이 심리검사에서 나타내는 양상들을 비교하고 ADHD와 우울장애를 진단하고 감별하는데 어떠한 시사점이 있는지 알아보기 위하여 실시되었다. ADHD로 진단된 아동 16명과 우울장애로 진단된 아동 14명을 대상으로 임상장면에서 전반적인 인지 기능을 평가하거나 장애를 진단하기 위한 정보를 얻기 위해 폭넓게 사용되고 있는 Wechsler 아동용 지능검사(K-WISC-III), ADHD를 진단하고 평가하기 위해 광범위하게 사용되는 연속수행검사(Continuous Performance Test)의 일종인 전산화된 주의력장애 진단시스템(ADS) 그리고 아동의 심리적 문제에 관한 정보를 수집하기 위해 아동용 인성검사(KPI-C)를 실시하였다. 그 결과 첫째, ADHD 아동과 우울장애 아동들은 지능검사에서 전체지능, 언어성 지능, 동작성 지능 및 지표점수, 소검사 점수에서 집단 간에 유의한 차이를 보이지 않아 지능검사 프로파일 분석은 ADHD와 우울장애를 변별하는데 민감한 요인이 되지 못하는 것으로 시사되었다. 둘째, ADS 시행 결과 ADHD 집단은 우울장애 집단에 비해서 시각과제의 오경보 오류가 유의미하게 높은 것으로 나타나 시각 ADS가 ADHD 집단의 주의력 문제를 예민하게 판별하는데 도움을 줄 수 있을 것으로 시사되었다. 셋째, K-PIC 시행결과 ADHD 집단은 우울장애 집단에 비해 과잉행동척도에서 유의하게 높은 점수를 나타낸 반면 여타의 척도들에서는 유의한 차이가 나타나지 않았다. 이러한 결과는 K-PIC 실시 시 우울장애와 ADHD를 변별하는 가장 유용한 측정치가 과잉활동척도임을 시사해준다. 연구 결과에서 나타난 바를 토대로 ADHD와 우울장애를 진단하고 변별하기 위해서 심리검사 결과를 해석할 때 주의해야 할 점과 연구의 시사점 및 제한점이 논의되었다.The purpose of this study was to understand clinical characteristics between a group of children with Attention Deficit Hyperactivity Disorder(ADHD) and the other group of children with Depressive Disorder. the participants were 16 ADHD children and 14 Depressive Disorder children. The cognitive characteristics were assessed using Korean-Wechsler Intelligence Scale for children-Third Edition (K-WISC-Ⅲ), ADHD Diagnostic System(ADS). The behavioral characteristics were assessed using Korean Personality Inventory for Children(KPI-C).
The results were as follows. First, the comparison of Full IQ scores, verbal IQ scores, performance IQ score, indexes scores of Verbal Comprehension, Perceptual Organization, Freedon from Distractibility, Processing Speed and subtest scores in K-WISC-III between ADHD group and Depressive Disorder group did not show significant differences. Second, In ADS, ADHD group showed significantly lower level of performance in commition error in visual ADS. Third. In KPI-C, ADHD group showed significantly higher score in Hyper Reaction than Depressive Disorder Group.
The findings suggest that performance in commition error of visual ADS and score in Hyper Reaction of KPI-C might be the useful variables to differentiate the ADHD group from Depressive Disorder Group.
