목적: 인공 슬관절 전치환술에서 슬관절의 정렬은 수술의 결과 및 예후에 많은 영향을 미치는 것으로 알려져 있다. 기존의 골수강외 정렬기법을 이용한 슬관절 전치환술은 술 중 대퇴골두의 중심을 확인하는데 한계가 있어 대퇴삽입물의 관상면 정렬이 정확하지 않은 문제점이 있었다. 본 연구에서는 골수강외 정렬기법을 위해 대퇴골두 중심간 거리를 이용하는 표식자의 정확성 및 대퇴 치환물의 관상면 정렬을 평가하고자 하였다. 대상 및 방법: 2008년 7월에서 2009년 7월까지 90명 100예의 슬관절을 대상으로 하였으며 술 전 PACS system에서 양측 대퇴골두 중심간 거리를 측정하였고 이를 새롭게 고안된 표식자에 적용하여 환자에 장착 후 단순 고관절 전후면 방사선 사진을 촬영하여 위치 오차를 계산하여 보정하는 방법을 이용하여 시행한 인공 슬관절의 정렬 정도를 대퇴삽입물의 내외반각을 측정하여 분석하였다. 결과: 술 전 시행한 방사선 촬영상 평균 대퇴골길이는 402.5±16.2 mm였고, 수술장에서 실시한 영상에서 대퇴골두와 표식자와의 평균 거리는 49.6±17.9 mm, 대퇴 골두 중심의 평균 오차는 3.78±3.14 mm였다. 술 후 시행한 방사선 사진상 90예가 관상면상 1도 오차 범위 안에 들었으며, 모든 예에서 2o 오차 범위 안에 들었다. 평균 역학적 축에 대한 대퇴삽입물의 내외반각은 89.9±1.5o (범위, 84.4-93.6o)였고, 96예에서 대퇴 치환물의 관상면상 정렬이 90±3o였다. 결론: 양측 대퇴골두 중심간 거리를 임상적으로 응용한 표식자의 정확성을 평가한 결과 기존의 대퇴골두 중심을 찾는 방법들보다 편리하고 정확한 결과를 얻을 수 있었고, 이러한 방법을 이용한 인공 슬관절의 술 후 정렬 또한 우수하였다.Purpose: Alignment is an important factor in the long-term success of total knee arthroplasty. In the total knee arthroplasty, the conventional extramedullary femoral alignment system has signifi cant error in femoral coronal alignment, since it is diffi cult to fi nd the femoral head center and it is time consuming to determine during the operation. The purpose of our study was to evaluate the accuracy of the newly-designed marker system for extramedullary femoral alignment that uses radiologic distance between the 2 femoral head centers. Materials and Methods: Between July 2008 and July 2009, 90 patients (100 knees) with knee osteoarthritis underwent total knee arthroplasty using the femoral extramedullary femoral guide system. We measured the distance between the femoral head centers using the radiologic picture archiving and communication system (PACS, General Electric, Milwaukee, WI) system preoperatively, then plastic rods and metal pegs were used to simulate the inter femoral head center distance. By placing the center of the plastic rod after marking the inter femoral head center distance on the central body line, we could trace the coronal mechanical axis. We measured the angle between the femoral mechanical axis and the femoral component in standing long leg antero-posterior radiograph to assess radiologically the accuracy of coronal alignment postoperatively. Results: The average femoral length in the study group was 402.5±16.2 mm. The mean distance between the femoral head and the center of the marker was 49.6±17.9 mm. The average error in estimation of the distance between the femoral head center and the metal peg of the marker was 3.78±3.14 mm. The positional error in alignment was 1o or less in 90% and 2o or less in 100% of knees. The average angle of femoral component to femoral mechanical axis was 89.9±1.5o (range 84.4-93.6o). The coronal alignment of the femoral components was within 90±3oin -96% of cases. Conclusion: Our results suggest that the clinical method reported here is a simple and reliable method to localize the center of the femoral head during total knee arthroplasty.
