목 적: 족관절 삼과 골절에서 후과 골절편의 크기가 관절면의 25% 이상을 차지하는 경우에 대하여 후외방 도달법에 의한 관혈적 정복 및 내고정술로 치료하여 2년 이상 장기 추시 후 그 결과를 분석하고, 그 치료 효과에 대하여 알아보고자 하였다.
대상 및 방법: 2004년 5월부터 2008년 4월까지 족관절 삼과 골절로서 본원에서 수술한 34명 중 후외방 도달법을 이용하고 2년 이상 추시가 가능하였던 20예를 대상으로 하였고, 평균 추시 기간은 34개월 (24∼58)이었다. 모든 예에서 술전 후과 골절편이 관절면의 25% 이상을 침범하였고, 11예에서 2 mm 이상의 전위를 보였다. 후외측 도달법을 이용하여 후과 골절편과 외과를 고정한 후, 방사선학적 양상에 대하여 분석하였고, AOFAS 점수를 통하여 기능 및 통증에 대한 평가를 하였으며, 합병증에 대하여 조사하였다.
결 과: 모든 환자에서 골유합을 얻었으며, 골유합까지의 기간은 평균 13.1주였다. 합병증으로 피부조직의 구축으로 인한 관절 운동제한 2예 (10%), 외상 후 관절염이 1예 (5%)에서 관찰되었고, 17예 (85%)에서 AOFAS 점수상 우수한 결과를 보였다.
결 론: 족관절 삼과 골절의 수술적 치료에 있어서 후외방 도달법을 이용한 방법은 족관절 외과 골절의 정복 및 고정 시 후과 골절편에 대하여 동시에 정복 및 고정이 가능하고, 장기 추시상에서도 좋은 결과를 보여 매우 유용한 접근법 중의 하나로 생각한다.Purpose: To analyze the long term follow up results of treatment with posterolateral approach and to investigate its usefulness in the patients of trimalleolar fracture with posterior fragment which is above 25% of articular involvement.
Materials and Methods: There were 34 cases of trimalleolar fracture in our hospital from May 2004 to April 2008. We investigated 20 patients who underwent operation with the posterolateral approach and over-2 years follow up cases. The mean follow up period was 34 (24∼58) months. Preoperative posterior malleolar fragment involved above 25% of articular surface in all cases and displaced more than 2 mm in 11 cases. We analyzed the radiologic type of posterior malleolar fragments and evaluated the function and pain through AOFAS score and complications.
Results: All cases showed primary union at mean 13.1 weeks. The complications are that partial ankylosis result of soft tissue contracture is seen in 2 cases (10%) and post-traumatic arthritis is seen in 1 cases (5%) and 17 cases (85%) of all patients are showed excellent AOFAS score.
Conclusion: The posterolateral approach is a valuable method because that it enables us to easily reduction and internal fixation of the posterior malleolus and lateral malleolus at one time and the results are satisfied for a long time follow up.
