Haddad 증후군은 선천성 중추성 무호흡 증후군에 선천성 거대 증이 동반되는 질환이다. 이는 신경 전구 세포의 이주 장애로 인해 발생하는 것으로 생각되고 있으며 PHOX2B 유전자의 돌연변이와 관련되어 있다고 알려져 있다. 저자들은 청색증과 장 폐쇄 증상으로 내원한 남아에서 선천성 중추성 무호흡증과 전장을 침범하는 거대 결장증이 병합되어있음을 진단하였고 PHOX2B 유전자의 27 다 염기서열 반복 돌연변이를 확인하였다. 환아에서 정상적인 연동운동을 보이는 장은 약 70 cm밖에 되지 않았다. 한국에서 PHOX2B 유전자 돌연변이를 확인한 Haddad 증후군은 이미 두 차례 보고된 바 있다. 그러나 두 증례 모두 26 다 염기서열 반복 돌연변이였으며 심각한 장애는 없었다. 본 저자들은 단장 증후군을 피할 수 없었던 27 다 염기서열 반복 돌연변이를 보인 Haddad 증후군을 경험하였기에 보고하는 바이다.Haddad syndrome is an extremely rare disorder which combines congenital central hypoventilation syndrome (CCHS) and Hirschsprung's disease. Several reports have demonstrated that CCHS was related to mutation of the PHOX2B gene. We report here a newborn male infant with apnea and bowel obstruction. He has been diagnosed with combined congenital central hypoventilation syndrome and Hirschsprung's disease, and had 27 polyalanine repeats in the PHOX2B gene. Two cases of Haddad syndrome with identified PHOX2B gene mutation have been recently reported in Korea. Both of them had extended alleles containing 26 polyalanine repeats. It is known that increased number of polyalanine repeat mutations is associated with a more severe clinical phenotype. The baby reported here had 27 alanine repeats (i.e. one more than the previously reported cases in Korea) and ganglion cells could be found only in the distal 70 cm of his small bowel.
