목적: 본 연구에서는 만삭 신생아 경련의 신경발달 예후를 알아보고, 임상 양상과 뇌파 및 뇌영상 검사 소견 중 예후에 관여하는 인자를 알아보고자 하였다. 방법: 1994년 6월부터 2007년 7월까지 가톨릭의대 여의도성모병원 신생아집중치료실에 입원한 만삭 신생아 경련 환자 의 신경발달 예후를 후행적 방법으로 조사하였다. 중증 신경발달 이상이 있는 군과 정상군으로 구분하여 신경발 달 예후와 관련된 위험인자를 분석하였다. 결과: 만삭 신생아 경련의 원인 질환 중 저산소성 허혈성 뇌증이 76.2%로 가장 흔하였다. 경련의 발생 시기는 생후 첫 24 시간 이내와 2-3일에 각각 60.0%, 23.6%로 대부분이 생후 3일 이내에 발생하였으며, 이들 중 67.4%에서 신경발 달 장애를 보여 생후 4일 이후에 발생한 경련보다 예후가 불량하였다. 비전형적 경련과 다국소 간대성 경련이 가 장 흔히 보인 경련이었으며, 경련의 유형과 신경발달 예후는 유의한 상관성이 없었다. 뇌파검사상 중등도 혹은 중 증 이상을 보였던 증례는 뇌파가 정상 혹은 경증 이상을 보였던 증례에 비하여 신경발달 예후가 통계적으로 유의 하게 불량하였다. 기타 신경발달 예후와 관련된 요인으로는 5분 아프가 점수, 뇌 자기공명영상 검사에서 저산소성 허혈성 뇌증 소견의 유무, Sarnat 병기, 뇌파 검사의 중증도, 경련 조절에 사용된 항경련제의 수, 뇌파소견의 정상 화에 소요된 기간 등이었다. 결론: 만삭 신생아 경련에서 뇌 자기공명영상 검사상 저산소성 허혈성 뇌증 병변과 Sarnat병기, 항경련제에 대한 반응성, 뇌파이상의 중증도는 신경발달 장애의 위험인자로 예후 예측에 도움이 될 수 있을 것으로 생각된다.Purpose: The objectives of this study were to observe the major neurodevelopmental sequelae of the full-term neonatal seizures, and to identify the risk factors associated with the poor neurodevelopmental outcomes. Methods: A retrospective review of the medical records of full-term newborns who had clinical and/or electrographic seizures in neonatal intensive care unit of St. Mary’s Hospital between June 1994 to July 2007 was performed. To assess the risk factors associated with poor neurological outcome, various factors were analyzed with univariate analysis and multiple regression analysis (SAS for Windows version 9.2). Results: The most common etiology of seizures in full-term infants was hypoxic ischemic encephalopathy (76.2%). The most common type of seizure was subtle (50.9%), followed by multifocal clonic (41.8%), and the seizure type had no significant correlation to the prognosis. Moderate to major EEG abnormalities were significantly related to poor clinical outcome. Additional factors related to neurodevelopmental outcome were Apgar score at five minute, evidence of HIE on brain MRI, Sarnat stages of HIE, number of anticonvulsant drugs used for seizure control and duration for normalization of EEG abnormalities. Conclusion: The risk factors observed in this study may be helpful to predict the neurological outcomes in full-term neonates with seizures.
