최근 현대인의 수면시간 단축이 체중 증가나 비만의 유병률과 밀접하게 관련되어 있다는 연구결과들이 일관되게 보고되고 있다. 특히, 전세계적으로 아동 및 청소년의 비만 문제가 중요한 공중보건학적 이슈로 대두되고 있어, 최근의 많은 연구들은 아동·청소년의 수면 부족과 이들에게서 증가하고 있는 비만 유병률과의 연관성에 대하여 더욱 주목하고 있다.
미국 국립수면재단은 8.5-9.25시간을 청소년기에 필요한 수면시간으로 권고하고 있지만, 질병관리본부가 발표한 2011년 청소년건강행태온라인조사에 의하면 대한민국 청소년의 대다수가 권고 수면시간에 미치지 못하고 있는 실정이다. 청소년들의 이러한 수면 부족은 우리나라에서 급속히 증가하고 있는 청소년의 비만 추이와 무관하지 않을 것으로 사료된다.
이에, 본 연구에서는 국민건강영양조사 제 4기(2007-2009)와 제 5기(2010-2011)에 모집된 13-18세 청소년 2,128명(남 1,156명, 여 972명)을 대상으로 하여 수면시간과 비만과의 관련성을 평가하였다. 대상자들은 최종적으로 연령에 따라 두 그룹(13-15세 1,290명, 16-18세 838명)으로 나뉘어 졌으며, 수면시간에 따른 따른 비만의 교차비(odds ratio: OR)를 로지스틱 회귀모델을 이용하여 검정하였다.
비만에 대한 정의는 질병관리본부에서 제정한 2007 소아 및 청소년 표준 성장도표를 이용하여, 체질량지수(body mass index: BMI)가 성별·연령별 체질량지수의 성장도표 대비 95 백분위수 이상이거나 백분위수와 무관하게 체질량지수가 25kg/㎡인 경우 고 위험 체질량지수(high BMI)로 판정하였다. 또한, 성별·연령별 허리둘레(waist circumference: WC)의 성장도표에 대비하여 허리둘레가 90 백분위수 이상이거나, 백분위수와 무관하게 남자는 90cm 이상, 여자는 80cm 이상일 경우 고 위험 허리둘레 (high WC)로 정의하였다.
수면시간은 국민건강영양조사의 건강설문조사 항목 중 "하루에 보통 몇 시간 주무십니까?"라는 질문에 대상자들이 직접 응답한 수면시간을 활용하여 5개의 카테고리로 분류하였다(6시간 미만, 6-6.9시간, 7-7.9시간, 8-8.9시간, 9시간 이상).
분석 결과, 조사년도, 연령, 성별, 가구소득, 흡연 유무, 음주 유무, 격렬한 신체활동량, 중등도 신체활동량, 부모의 비만여부, 스트레스 정도, 총 에너지 섭취량을 보정한 후, 16-18세 청소년들의 경우 수면시간이 짧을수록 고 위험 체질량지수(p for trend 〈 0.001)와 고 위험 허리둘레(p for trend = 0.02)의 유병률이 더 높은 것으로 나타났다. 6시간 미만의 수면시간을 기록한 16-18세 청소년들은 8-8.9시간(reference group)의 수면시간을 기록한 청소년들에 비해 고 위험 체질량지수의 유병률이 3.3배 (95% CI = 1.55-7.04) 더 높은 것으로 관찰되었다. 반면, 13-15세 청소년들의 경우는 8-8.9시간을 기록한 청소년들에 비해 7-7.9시간과 9시간 이상의 수면시간을 기록한 청소년들에게서 고 위험 허리둘레의 유병률이 상대적으로 더 낮은 것으로 나타났다.
수면시간이 짧은 청소년들은 상대적으로 수면시간이 긴 청소년들에 비해 가구소득이 더 높았으며, 스트레스 정도가 더 심한 것으로 관찰되었다. 총 에너지 섭취량은 수면시간에 따라 차이를 보이지 않았지만, 밤 11시부터 오전 7시 이전(23:00-06:59)까지의 에너지 섭취량은 수면시간이 긴 청소년들에 비해 수면시간이 짧은 청소년들에게서 상대적으로 더 높았다. 또한, 그들은 대체로 가족들과 식사를 잘 하지 않는 것으로 나타났으며, 16-18세 청소년의 경우 수면시간이 짧은 그룹에서 탄수화물 섭취량과 탄수화물로부터의 에너지 섭취 비율도 더 높은 것으로 관찰되었다.
결론적으로, 본 단면조사 연구는 수면시간과 비만과의 시간적 선후관계를 분명히 제시할 수는 없었지만, 한국의 청소년 집단을 대표할 수 있는 데이터를 이용하여 수면시간과 비만 간의 유의한 상관관계를 도출하였다. 16-18세의 청소년들에게서 관찰된 수면시간과 비만과의 음의 상관관계는 수면시간이 짧을수록 비만의 위험이 증가한다는 기존의 연구들과 그 결과를 같이 하며, 만성적인 수면 부족이 체중 증가나 비만을 유발할 수 있다는 기존의 가설을 지지하고 있다. 따라서, 향후 연구에서는 수면시간과 비만과의 선후관계를 명확하게 밝힐 수 있는 전향적 연구가 진행되어야 할 것이며, 아울러 건강한 수면습관과 충분한 수면시간의 중요성을 권장하는 수면교육을 아동·청소년을 대상으로 한 비만 교육 프로그램에 포함시키는 방안을 강구할 것을 제안한다.
