거동이 불편하거나 문제행동을 하는 노인을 보살피는 일은 육체적?정신적 부담이 따르며 때로는 우울감을 불러오기도 한다. 이러한 정신적 반응은 인종과 문화에 따라 다르게 나타나는데 한국 간병인들은 환자의 기능 장애나 문제행동이 미국 거주 한인 환자에 비해 적음에도 불구하고 간병 부담감과 우울감이 미국의 한인 간병인에 비해 높은 것으로 알려져 왔다. 본 연구는 사회문화적 스트레스와 적응모델을 이용하여 “효”가치관이 간병인의 적응 양식과 우울감에 미치는 영향에 대해 연구하였다. 87명의 미국 거주 한인 가족 간병인들과 83명의 한국 거주 가족 간병인들을 면접 설문조사하였다. 경로 분석을 통한 연구 결과와 제언은 다음과 같다.
한국의 간병인들은 미국의 한인 간병인과 비교하여 높은 간병 부담감과 우울감을 보이는데, 이는 그들이 사용하는 적응양식 차이로 설명될 수 있다. 본 연구에서는 능동적 적응 대처 양식은 간병인의 우울감 경감 효과가 있는 것으로 밝혀졌는데, 한국의 간병인들은 미국의 한인 간병인에 비해 능동적 적응 대처 양식을 적게 사용하고 수동적?사회의지 적응양식을 더 많이 사용하였다. 수동적?사회의지 적응양식은 간병 부담감이나 우울감에 영향을 미치지 않았다. 따라서 간병인 교육에 있어서 능동적 적응 대처 양식을 보다 잘 활용할 수 있도록 하는 한편 수동적?사회의지 적응 양식이 유용한 적응양식이 아닌 점을 인식시켜야 할 것이다. 또한 “효”가치관은 능동적 적응양식에 긍정적 영향을 미치는 것으로 나타났다. 향후 “효” 가치관과 능동적 적응양식, 그리고 “효”가치관의 긍정적 영향에 대한 연구가 보다 활발히 이루어져야 할 것이다. 더 나아가, 환자의 거동 불편 또는 문제 행동 정도가 커짐에 따라 간병인의 부담감과 우울감이 함께 증가되는 것으로 나타나 이를 완화시킬 수 있는 실제적인 정책방안들이 마련되어져야 함을 제언하였다.
Using the Sociocultural Stress and Coping Model (Aranda & Knight, 1997), this study explores the effects of filial piety values on caregiver burden and depression. Eighty-seven Korean-American caregivers and 83 Korean caregivers were interviewed using questionnaires. The results of path analysis and discussion are as follows.
Korean caregivers used less active coping and more passive coping compared to Korean-American caregivers. The use of active coping was found to be related to lower caregiver depression. However, Korean caregivers used more passive coping which showed no significant effect on caregiver burden and depression. With regards to caregiver training, it should be emphasized that active coping, but not passive coping, helped reduce caregiver distress.
In addition, filial piety values showed a positive effect on active coping. More studies of filial piety and active coping are recommended. It is also necessary for the "moderners" who view traditional values with less regard, to pay attention the positive effects of filial piety values. Furthermore, we should prepare policies to reduce the negative effects of caregiving. Higher levels of functional difficulties and behavior problems correspond with higher levels of caregiver burden and depression.권호기사
