스트레스에 유발되는 긴장성 두통은 매우 흔한 두통의 하나이다. 본 연구의 목적은 스트레스와 연관되어 경부근긴 장이 동반된 두통 환자들에서 정상인에 비하여 평형기능에 어떤 이상이 있는지를 정량적으로 알아보고자 하였다. 스트레스로 촉발된 긴장성 두통으로 진단받은 환자 30명을 대상으로 하고, 정상인들을 나이와 성별로 짝지어 대조군으로 하였다. 설문지로 스트레스를 평가하고 전정유발근전위검사(VEMP)가 포함된 전정기능검사 및 자세측정기 검사를 시행하였다. 결과로 스트레스성 긴장성 두통 환자들이 대조군보다 스트레스가 많았으며, VEMP 진폭은 두통 환자군이 대조군보다 통계적으로 유의하게 저하되었다. 두통 환자들의 압력의 중심 동요 궤적은 1.0 Hz 이하의 동요 주파수영역에서 파워가 대조군보다 유의하게 컸다. 전산화동적자세 측정기를 사한 감각분석에서 체성감각비는 두통환자군이 유의하게 저하되었다. 이로 보아 스트레스성 긴장성 두통 환자들은 경부근긴장에 동반되어 미로 기능 및 전정둔덕반사에 이상이 있거나 고유감각에 이상이 있을 수 있음을 알 수 있었다. 또한 두통환자의 평형기능 이상 이 스트레스로 인해 말초성 병변 뿐 아니라 중추성 병변에서 왔을 가능성을 제기하는 결과로 사료된다

Stress-induced tension-type headache (STH) is one of the most common types of headaches. They can be brought on- or triggered by stress and muscle strain. The purpose of this study was to investigate whether patients with STH have abnormalities of postural control, and to disclose any frequency shift of the centre of gravity while standing upright. Thirty patients with STH diagnosed by the International Headache Society Classification criteria and with tenderness in pericranial muscles were compared with age and sex matched controls. The questionnaires for stress amount were asked to both groups. All the subjects underwent balance measurements that included vestibular evoked myogenic potential (VEMP) testing, static and computerized dynamic posturography. Stress amount were significantly increased in the patient group. STH patients had reduced VEMP amplitudes compared to the controls. The spectral frequency analysis of body sway while standing upright was investigated. The sway distance and the sway area of center of foot pressure significantly increased in the patients with STH. Spectral frequency analysis showed a significant increase in spectral power below 1.00Hz frequency band. Computerized dynamic posturography showed decreased somatosensory ratio in the patients with STH. These findings suggest that stress may induce abnormal vestibulo-collic reflex and a significant proprioceptive disturbance in tension headache patients.
