목적: 아래눈꺼풀의 무통성 종괴를 주소로 내원한 환자에서 원발점액샘암종을 경험하였기에 이를 보고하고자 한다.

증례요약: 62세 남자 환자가 내원 2개월 전부터 발생한 왼쪽 아래눈꺼풀의 크기가 증가하는 무통성 종괴를 주소로 내원하였다. 전산화 단층촬영과 안와 자기공명영상검사에서 왼쪽 눈꺼풀에 약 2 cm 크기의 경계가 명확한 원형의 균일하게 조영되는 종괴가 확인되었다. 정확한 진단 및 치료를 위하여 전신마취하 종양 절제생검을 시행하였고 병리조직검사에서 점액샘암종으로 확진되었다. 몸통 양전자 방출단층촬영 및 전산화단층촬영에서 이상 소견을 발견할 수 없어 원발점액샘암종으로 최종 진단하였고 종양의 완전 절제를 위해 광범위 절제술을 시행하였다.

결론: 눈꺼풀에 발생하는 원발점액샘암종은 매우 드물게 발생하는 악성 종양으로, 전이 여부를 감별하기 위한 전신적인 검사가 반드시 필요하며, 국소 재발률이 높기 때문에 광범위 절제 후 정기적인 추적 관찰이 요구된다.

Purpose: We report a case of a primary mucinous adenocarcinoma of the lower eyelid diagnosed in patient with a painless mass.

Case summary: A 62-year-old male visited our clinic with a painless mass of the left lower eyelid of increasing the size for 2 months. Orbital computed tomography and magnetic resonance imaging revealed an approximately 2 cm well-defined ovoid, homogeneous enhancing mass in the left lower eyelid. A biopsy was performed under general anesthesia, and histopathologic examination confirmed mucinous adenocarcinoma. Final diagnosis was made as primary mucinous adenocarcinoma since torso positron emission tomography and computed tomography found no abnormal findings and extensive resection was performed for complete resection of the tumor.

Conclusions: Primary mucinous adenocarcinoma that occurs in the eyelid is a very rare malignant tumor, and a systemic examination to determine whether it is metastatic is essential, and due to its high local recurrence, regular follow-up after extensive resection is required.
