The purpose of this study was to suggest an efficient hospital management strategy by identifying the characteristics of patients with industrial accident compensation insurance. For over two years and seven month, from March 1, 2009 to September 30, 2011, this survey was performed targeting at a total 275 injured cases(includes 210 patients) after discharging during that period, and the results of analysing data are as follows:
First, the distribution of workplaces of industrial disaster was followed by Gyeonggi-do with 43.64% the highest, Seoul with 29.82%, and other areas with 26.54%, and of the type of the business aspects, 38.18% the highest in construction industry, 37.82% in other industries, and the manufacturing industry was followed by 17.45%.
Second, of the number of medical care and health care revenue aspects, the (joint) orthopedic showed the highest level with 43.64% of the overall number of medical care and 35.33% of the total health care revenue. Then the (spine) neurosurgery and (stroke) neurosurgery are followed next by 10.55% each in the number of medical care, and (spine) neurosurgery was 16.33% and (stroke) neurosurgery was 13.60% in health care revenue.
Third, of the average daily medical cost aspects, (cardiovascular) cardiovascular medicine was revealed the highest level with \771,228, and followed by (respiratory) respiratory medicine, (gastroenterology) surgery, (otorhinolaryngology) otolaryngology, (stroke)neurosurgery, (spine) neurosurgery, and (spine) orthopedics. (Cardiovascular) cardiovascular medicine, (respiratory) respiratory medicine (gastroenterology) surgery, and (otorhinolaryngology) otolaryngology are exceptional circumstances, and thus (stroke) neurosurgery, (spine) neurosurgery, (spine) orthopedics are in the top three.
Fourth, in comparisons of the average length of hospitalized days in patients with a health insurance and industrial accident compensation insurance, 17 departments besides rehabilitation medicine were comparatively analysed and at the 15 departments, except three departments, the industrial accident compensation insurance appeared to be as few as one time and as many as five times in the average length of hospitalization.
Fifth, when compared according to the classified additional rates of the medical institution, the industrial accident compensation insurance was turned up to have additional 15% in a tertiary hospital, 12% in a general hospital, 1% in a hospital. As above, the industrial accident compensation insurance are based upon applying the higher classified additional rates for medical treatments than the health care insurance. Of estimating hospital expenses aspects, the industrial accident compensation insurance alleviated the period of sentiment or excluded sentiment application according to the different medical institutions in contrast with the health care insurance.
Lastly, when comparing the current status of entire industrial disasters with that of a general hospital of Korea in 2010, the accident patterns showed that the current status of entire industrial disasters were followed by fall, being pulled, crash, while crash, traffic accident, fall were followed in a general hospital. The study represented the same results of higher disasters at the workplaces with less than five workers and under six months working period.
On a basis of the results of this study I propose as follows:
First, as the types of business in patients with industrial accidents aspects, construction industry was the highest with 38.18%, other industries were 37.82%, and manufacturing industry was 17.47%. I suggest to make a business agreement with above industries. If they use a general hospital not only as an industrial disaster but also as a patient with a health care insurance or as a general patient, it will be an advantage in the hospital management.
Second, the method to reduce the length of hospitalization should be considered. In comparisons of the average length of hospitalization between the health care insurance and the industrial accident compensation insurance, 15 departments, except three departments, showed that the industrial accident compensation insurance appeared to be as few as one time and as many as five times of the health care insurance by the results of analysis on 17 departments besides rehabilitation medicine. These results will have a bad influence on the hospital management. Therefore, I propose to draw up a plan in order to decrease the length of hospitalization in conjunction with the staff in the department of the industrial accidents and medical sector.
