배경 및 목적:Sirolimus 용출성 스텐트는 염증반응을 억제하고 신생내막의 형성을 억제하여 재협착률을 줄인다. 본 연구는 스텐트 재협착 환자에서 Sirolimus 용출성 스텐트의 안정성과 추적 관상동맥 조영술상의 성적을“커팅 벌룬” 혈관성형술과비교하여 평가하고자 하였다.방 법:Sirolimus 용출성 스텐트를 시행받은 스텐트 재협착을 가진 41명의 환자(43병변)를 동일 병변을 가진“커팅 벌룬”혈관성형술을 시행받은 57명의 환자(61병변)와 후향적으로비교하였다. 시술의 안정성과 추적 기간동안의 주요 심장사건의 유무, 추적 관상동맥 조영술을 통하여 재협착의 빈도,최소병변 내경과 조기이득, 후기소실 등을 비교 분석하였다.
결 과:
연령, 성별, 임상 진단, 위험인자, 병변혈관의 분포, ACC/
AHA 분류에 의한 병변의 분류, 좌심실 구혈율, 재협착의
형태에 의한 분류등은 차이가 없었다. 관상동맥 중재술은 양
군에서 전 예에서 성공하였으며, 시술과 관련된 합병증은 없
었다. 추적 기간 동안 사망 및 급성 심근경색은 양 군에서
없었으나 목표 혈관 재관류술은 SES 군에서는 없었고, CBA
상 재발성 재협착은 SES 군에서는 없었고, CBA 군에서 16
은 SES 군 11.3±9.6, CBA 군 41.2±24.8%로 SES 군에
소실지수(loss index)는 SES 군 0.08±0.10, CBA 군 0.36
결 론:
Sirolimus 용출성 스텐트는 시술 관련 합병증과 유의한 부
작용 없이 스텐트 재협착의 치료에 사용될 수 있으며 주요 심
장사건의 발생없이 재발성 재협착률을 획기적으로 줄였다.Background and Objectives:The use of drug-eluting stent implantation for in-stent restenosis (ISR) has shownsome promising results. This study investigated the clinical and angiographic results of performing sirolimuselutingstent (SES) implantation for the treatment of patients with ISR as compared with treatment with cuttingballoon angioplasty (CBA). Subjects and Methods:Forty one patients with ISR (43 lesions) were treated withSES implantation and they were retrospectively compared with a group of patient with matched lesions that weretreated with CBA (57 patients with 61 lesions). Routine angiographic follow-up was obtained at about 6 monthsafter treatment and the incidence of major adverse cardiovascular events was evaluated. Results:The baselineclinical characteristics, the patterns of ISR and the minimal luminal diameters (MLD) were not different betweenthe two groups. There was no procedural failure and no immediate major cardiovascular adverse events (MACE)in the two groups. There were two target lesion revascularizations during the clinical follow-up in the CBA groupand no MACE was found in the SES group (4% vs. 0%, respectively p<0.001). On the follow-up angiogram, theMLD and the stenosis diameter were significantly lower in the SES group than in the CBA group (2.82±0.33 vs.1.80±0.82 mm, p<0.001; and 11.3±9.6 vs. 41.2±24.8%, p<0.001, respectively). The acute gain was significantlyhigher and the late loss was significantly lower in the SES group than in the CBA group (2.64±0.38 vs.2.02±0.44 mm, p<0.001; and 0.21±0.28 vs. 0.71±0.73, p<0.001, respectively). Conclusion:The sirolimuselutingstent was safe, feasible and highly effective for treating the patients suffering with in-stent restenosis ascompared with cutting balloon angioplasty.
