배경 및 목적:PMV는 MS 환자에 있어 안전하고 효과적인 방법으로 알려져 있으나, MS의 판막의 개구 면적의 중증도에 따른 시술의유용성은 잘 알려져 있지 않다. 본 연구는 중등도 및 중증 MS에서 PMV 시술의 장기 유용성을 비교하여, 중등도 MS에서조기 PMV시술의 유용성에 대하여 알아보고자 하였다.방 법:1988년 4월부터 2000년 3월까지 연세대학교 신촌 세브란스 병원에서 PMV를 시행 받은 786명(남자 198명, 평균 나이 42세)의 임상 경과 및 심초음파 결과를 중등도(n=357)와중증 MS 환자(n=429)에서 비교 분석하였다.
결 과:
PMV 시술 결과는 전체 환자 중 578(78%)에서 성공적이었
으며, 중등도 환자에서의 성공률(291명, 82%)이 중증 환자들
(327명, 76%)에 비하여 높은 경향을 보였다(p=0.08). 시술 직
후 사망은 없었으며, 급성 합병증은 85예(11%)에서 나타났다.
평균 추적 관찰 기간은 94개월(6~210개월)이었고, Eventfree
survival은 5년에는 중등도 MS(88%)에서 중증 MS(79%)
보다 높았지만 10년에는 양 군 모두에서 65%로 차이가 없
었다. 장기 event-free survival에 대한 시술 전 예측인자로
는 8점 이하의 낮은 심초음파 점수(p=0.017, HR=0.67), 시
술 후에는 1.7 cm2 이상의 넓은 승모판 개구 면적(p=0.002,
HR=0.65), 시술 후 2도 이하 MR(p=0.001, HR=0.46)과 시
술 후 12 mmHg이하의 좌심방압(p=0.005, HR=0.80)였다.
결 론:
PMV는 중증 MS와 중등도 MS 간에 단기 및 9년 이상의 장
기 시술 성적은 차이가 없었다. 따라서 모든 중등도 MS 환
자에서의 PMV 시술은 적합치 않으며 특히 8년 이내에서 효
과가 필요한 노인이나 임산부 등의 선택된 환자에서 시행되
는 것이 바람직하리라 생각된다.Background and Objectives:Percutaneous mitral balloon valvuloplotomy (PMV) is a safe and effective procedurefor the treatment of mitral stenosis (MS); however, its long-term results according to the severity of MSremain unknown. The aim of this study was to compare the long-term results of PMV between moderate andsevere MS. Subjects and Methods:The immediate and long-term outcomes of 786 patients (198 male, mean age;43 years) who underwent PMV at a single referral center, between 1988 and 2000, were analyzed. The clinical andechocardiographic data were compared between moderate (n=357) and severe MS (n=429). Results:Optimalresults were gained in 618 (79%) patients, including 80 and 76% moderate and severe MS cases, respectively(p=0.13). Acute complications occurred in 85 patients (11%), but without significant difference between the twogroups. After a mean follow-up period of 94 months (range, 6 to 210 month), the 5 year event-free survival ratewas better in moderate (88%) than in severe MS (79%), but the 10 year event-free survival rates were no different(65% vs. 65%).
