신경펩티드인 Substance P (Sub P)는 11개의 아미노산으로 구성된 물질로서 신경계통에서 신경전달물
질 또는 신경조정물질로 작용하고 있으며, 또한 잠재적인 혈관성장물질로서 기능을 하고 있는 것으로 알려져 있
다. 본 연구는 흰쥐를 실험동물로 하여 중추신경계통의 연장부위이며 일차 시각 기관인 망막에서 Sub P의 발현
양상을 관찰하고 실험적으로 유도된 당뇨병 상태에서의 변화를 관찰함으로써 정상 망막에서 Sub P의 기능과 당
뇨병성 망막증 병인 기전 관련 여부를 규명하고자 하였다. Sprague-Dawley계 흰쥐를 실험돌물로 하여,
streptozotocin의 정맥내 주사로 당뇨를 유발시켰으며, 공복 혈당 300 mg/dL 이상의 동물을 각각 1, 4, 8 및 12주
경과시켜 당뇨병 실험 군으로 사용하였다. 정상 및 당뇨병 망막에서 Sub P 발현 및 변화 양상을 면역조직화학법
및 RT-PCR을 통하여 관찰 분석하였다.
정상 흰쥐 망막에서 Sub P 면역반응성은 속핵층의 무축삭세포, 신경절세포층의 전위무축삭세포 및 이들의 세
포질돌기가 분포된 속얼기층에서 나타났다. 속핵층에 위치한 무축삭세포의 세포질돌기는 속얼기층의 제1, 3, 5층
에 분포하였고 신경절세포층에 위치한 전위무축삭세포의 세포질돌기는 주로 속얼기층의 제5층에 분포하였다.
망막 전체봉함표본의 면역염색을 통해 Sub P 면역반응 전위무축삭세포의 수를 측정한 결과 위-관자 부위에 가
장 많은 수가 분포되어 있었으며, 아래-코 부위에서 가장 적게 관찰되었다. 당뇨병 망막에서 속핵층에 존재하는
Sub P 면역반응 무축삭세포 수는 정상 동물군과 비교해볼 때 당뇨 4주군에서 가장 많이 감소하였으며, 그 이후
점차 증가하는 양상이었다. 또한 신경절세포층에 존재하는 Sub P 면역반응 전위무축삭세포 수도 당뇨 4주군에서
감소하였고, 당뇨 12주군에서 증가하는 경향이었음을 확인하였다. Sub P의 양적 증감을 알아보기 위해서 RTPCR을
수행한 결과, sub P의 mRNA 발현 수준은 정상 수준과 비교했을 때 당뇨 4주군에서 40% 정도 감소하였
으며, 당뇨 12주군에서 정상 수준 대비 80% 정도로 증가하였다.
이상의 결과로써 흰쥐 망막에서 Sub P는 무축삭세포와 전위무축삭세포에서 발현되어 망막 내에서 이루어지
는 일차적 빛 자극전달과정의 조절인자로 시각정보 처리과정에 관여하되 망막의 부위에 따라 달리 기능을 할
것으로 생각되며, 당뇨 상해에 의하여 비교적 초기에 Sub P 합성 능력이 손상을 받음으로써 당뇨병 속망막의 신
경퇴행성 변화에 기여할 것으로 추정된다.Substance P (Sub P) being composed of 11 amino acids sequence is a kind of tachykinin family
peptides. It has been known that this substance plays a role of neurotransmitter and/or neuromodulator and is a very
potent vascular growth factor in the nervous system. This study has been investigated expression pattern of Sub P in
the rat retina at normal and alteration of Sub P expression following diabetic injury using immunohistochemistry.
Diabetic condition was induced by a single injection of streptozotocin in Sprague-Dawley rats aged 8 weeks. The
animals showing high blood glucose levels (above 300 mg/dL) were cared for 1, 4, 8 and 12 weeks, respectively. The
whole-mounted or sectional preparations of the retinas were used for Sub P immunohistochemistry.
Sub P immunoreactivity has been localized in subsets of amacrine cells in the inner nuclear layer (INL) and
displaced amacrine cells in the ganglion cell layer (GCL) in the normal retina. The dendrites from amacrine cells in the
INL were ramified with strata 1 and 3, and those from displaced amacrine cells in the GCL with strata 5 of the inner
plexiform layer. Sub P immunoreactive neurons in both the INL and the GCL were more densely populated in the
superior half of the retina.
During diabetes, the cell number of Sub P immunoreactive neurons was decreased to one third of the normal value at
4 weeks of diabetes and then slightly increased to half of the normal value at 12 weeks of diabetes. In addition, Sub P
mRNA levels were reduced at 4 weeks but reincreased at 12 weeks.
These results suggest that Sub P in the rat retina at normal state may function differentially in the superior or the
inferior halves and Sub P synthetic pathway in the retinal neurons maybe irradiated in earlier stages of diabetic
retinopathy.
