배경 및 목적:고혈압 전단계는 수축기 혈압 120~139 mmHg, 이완기 혈압 80~89 mmHg으로 정의되며, 고혈압 예방을 위해 생활양식의 변화가 추천된다. 고혈압 전단계 군에서 정상혈압군보다 심전도와 심초음파 상 좌심실 비대가 더 높은 유병율을보이는지와 좌심실 이완 기능이 감소되어 있는지를 알아보고자 하였다.방 법:고혈압, 당뇨, 그 외 심혈관 질환의 과거력이 없는 40세이상 정상혈압군 335명과 고혈압 전단계군 273명에서 심전도 검사와 심초음파 검사를 시행하였다.
결 과:
고혈압 전단계 군은 정상혈압 군에 비해 심전도상 소콜로
우-라이언(Sokolow-Lyon) 전압과 코넬(Cornell) 전압이 유
의하게 더 높았고, 심전도상 좌심실 비대의 유병율이 높았다
(16.9% vs 5.9%). 심초음파 검사에서 고혈압 전단계 군은 정
상혈압군 보다 더 큰 좌심실 질량 지표(남: 118.1±24.4 g/m2
vs 102.0±19.1 g/m2, p=0.02; 여: 117.5±26.4 g/m2 vs
101.9±30.7 g/m2, p=0.02)를 보였고, 승모판 혈류 도플러
검사에서 E/A비는 고혈압 전단계 군에서 더 작은 값을 보였
다(남: 1.14±0.6 vs 1.30±0.4, p=0.03; 여: 1.11±0.6 vs
1.25±0.5, p=0.03).
결 론:
고혈압 전단계 군은 정상혈압 군에 비해 심전도상 좌심실
비대의 유병율이 높았으며, 심초음파 검사에서 좌심실 질량
지표가 증가되어 있었고, 좌심실 이완기능의 저하가 동반되어
있었다.Background and Objectives:We investigated whether prehypertensives (PHs) exhibit more prevalent electrocardiographicand echocardiographic left ventricular hypertrophy (ECG LVH) and if they are are more associatedwith impaired left ventricular (LV) diastolic function than are normotensives (NTs). Subjects and Methods:Atotal of 608 subjects≥40 years of age with normal BP (SBP<120 mmHg and DBP<80 mmHg, n=335) and PH(n=273) and who were without a history of hypertension, diabetes and any other known cardiovascular diseasesunderwent blood sampling for obtaining the lipid profile, and they also underwent 12-lead electrocardiographyand echocardiography. Results:Compared with the NTs, the PHs had significantly higher Sokolow-Lyon andCornell voltage for ECG LVH, and they ahd a higher prevalence of ECG LVH (men: 16.9% vs 5.9%, women:2.0% vs 1.0%, respectively). The PHs had a more increased left ventricular mass index (LVMI, Men: 118.1±24.4 g/m2 vs 102.0±19.1 g/m2, p=0.02; Women: 117.5±26.4 g/m2 vs 101.9±30.7 g/m2, p=0.02) and a moredecreased LV diastolic function (E/A, Men: 1.14±0.6 vs 1.30±0.4, p=0.03; Wemen: 1.11±0.6 vs 1.25±0.5,p=0.03) on echocardiography than the NTs. Conclusion:The PHs were associated with a higher prevalence ofECG LVH and a more increased LVMI on echocardiography than that of the NTs. Therefore, we should payearlier attention to diagnose and screen the pre-hypertensive group. (Korean Circulation J 2006;36:437-442)
