만성신부전의 뇨독증이 중추신경계의 이상을 일으키는 기전은 다인성이며 일부분만이 밝혀져 있다.
흰쥐 신장의 5/6를 절제하여 만든 만성신부전모델로 신경세포의 가지돌기에 존재하는 이온통로 단백질과 이들
의 연관단백질 PSD-95에 미치는 영향을 조사하였다.
본 실험의 결과에 의하면 만성신부전 후 NMDA와 AMPA 이온수용체를 싸고있는 가지돌기의 PSD-95 단백질
의 양은 의미있게 증가하였으나 NMDA와 AMPA 이온수용체의 아단위들의 정량적인 변화는 관찰되지 않았다.
따라서 만성신부전 후 초래되는 가지돌기의 PSD-95의 증가는 - NMDA와 AMPA 수용체들의 기능 향상을 통해
연접후 신경세포의 세포질 내 칼슘의 증가를 유도할 수 있으며 이는 연접후 신경세포에 기능적 손상을 가져올
수 있는 것으로 생각된다. 또한 신경세포 축삭에서의 신경미세섬유(NF200)의 감소는 세포골격의 손상으로 인해
세포내물질의 이동을 제한하고 이로 인해 세포의 기능에는 더욱더 심한 장애를 초래할 수 있을 것으로 생각된
다. 결론적으로 만성신부전으로 인한 가지돌기의 PSD-95의 증가는 신경세포들간의 기능적 이상을 초래함으로
서 기억의 감소를 유발시킬 것으로 생각된다.The mechanism of central nervous system (CNS) dysfunction in uremia are multifactorial and only
partially characterized. Studies using hippocampal formation (HF) evaluate the relationship between the uremia and
memory impairment. Immunoblots with calcium permeable NMDA (N-methyl-D-aspartate) and AMPA (2-amino-3-
hydroxy-5-methylisoxazole-4-propinoic acid) receptors and their associated PSD-95 proteins after chronic renal failure
(CRF) provided significant new informations.
CRF rats induced by 5/6 nephrectomized had significant effects on up-regulation of PSD-95 protein rather than
those of calcium permeable NMDA and AMPA receptor subunits. Up-regulation of PSD-95 after CRF might be
associated with the enhanced activity of NMDA and/or AMPA receptors, thereby leads to the intracellular Ca2+
accumulation and functional neuronal cell damage subsequently. Degradation of intermediate filament 200 (NF200) in
the axon after CRF may induce an impairment of intracellular transport and eventual cellular dysfunction through
destruction of the neuronal cytoarchitecture. These data suggest that up-regulation of PSD-95 in CRF may increase the functional derangement between the nerve cells and ultimately lead to memory impairment.
