배경 및 목적:
Mycophenolate mofetil(MMF)와 Interleukin-2 단일항
체(IL2mAb)를 이용한 면역억제제 병합요법은 심장이식환
자에서 거부반응을 효과적으로 억제하는 것으로 알려져 있
다. 하지만, 한국인을 대상으로 한 임상 연구는 없는 실정
으로 저자들은 심장이식을 받은 한국인을 대상으로 하여
이들 약제를 이용한 면역억제제 병합요법의 효과와 안정성
에 대해 알아보고자 하였다.
방 법:
1992년 11월부터 2003년 12월까지 111명의 환자가 본원
에서 심장이식을 받았고, 이들을 MMF와 IL2mAb를 포함
한 면역억제제 병합요법을 시행한 군(A군, n=51)과 이들
약제가 포함되지 않은 면역억제제 병합요법을 시행한 군(B
군, n=60)으로 나누어, 심장이식 후 2년 동안의 급성 거부
반응의 발생, 주감염의 발생률, 그리고 생존율 등을 비교하
고자 한다.
결 과:
연구기간 중 MMF에 의하여 5명의 환자에서 위장관계
부작용이 발생하였고, 1명이 약물을 중단하였다. 그러나,
IL2mAb에 의한 싸이토카인 유리 증후군이나 알레르기 반
응은 관찰되지 않았다. A군에서 cyclosporine 농도(212.3±
었음에도 불구하고, 2년 경과관찰기간 중, 치료를 요하는
었고, 심장이식 후 거부반응 발생까지의 시간 또한 A군이
러나, 두 군간의 2년 생존율은 A군 94%, B군 88%였고, 항
생제 치료가 필요했던 주감염의 발생률은 A군 26%, B군
20%로 모두 통계학적 유의성을 보이지 않았다.
결 론:
MMF와 IL2mAb을 이용한 면역억제제 병합요법은 심장
이식을 받은 한국인에서 효과적이고 안전하게 이식심장에
대한 급성 거부반응을 억제할 수 있을 것으로 사료된다.Background and Objectives:An immunosuppressive regimen including the use of mycophenolate mofetil (MMF)and an interleukin-2 monoclonal antibody (IL2mAb) has shown promise to prevent acute rejection after hearttransplantation. There has been a lack of report on the evaluation of the efficacy and safety of this regimen inpatients receiving heart transplants in Korea. Subjects and Methods:From November 1992 to December 2003,111 consecutive patients who had received heart transplants in our institute were classified into two groups: patientswho received the immunosuppressive regimen with MMF and an IL2mAb (group A, n=51) and patientswho did not receive the regimen (group B, n=60). We compared the clinical outcomes of patients in each groupincluding the survival rate and the occurrence of acute rejection and infection at 24 months post transplantation.Results:Both drugs were tolearated in all patients except in 5 patients who complained of gastrointestinal sideeffects due to MMF. Despite a longer ischemic time (137.4±54.6 vs. 92.3±25.8 hours, p<0.05) and a lower serumlevel of cyclosporine (212.3±66.8 vs. 259.1±62.1 ng/mL, p<0.05), the rate of treatment for acute rejectionwas lower in group A than in group B (16% vs. 53%, p<0.05). In addition, the median time to the first treatmentfor acute rejection was almost twice as long for group A as for group B (91 vs. 43 days, p<0.05). The 2-yearsurvival rate and the incidence of major infection requiring hospitalization in both groups were 94% vs. 88%and 26% vs. 21%, respectively, which were not statistically different. Conclusion:An immunosuppressive regimenincluding the use of MMF and an IL2mAb is efficacious and safe as a prophylaxis against acute rejectionwithout the increased risk of major infection in patients who have received heart transplants in Korea.
