Nestin은 중간세사에 속하는 단백질로, 줄기세포의 표지자로서 인지되고 있다. Nestin은 발생단계에서
는 매우 많이 발현하지만, 성체에서는 일부 세포에서만 발현된다. 그러나 질병 또는 손상 후에는 발현이 증가하
는 것으로 알려져, 조직의 재생 또는 세포의 생존에 관련되어 활성화되는 세포에서 기능을 하는 것으로 알려져
있다. 본 연구는 흰쥐의 콩팥에서 허혈-재관류 손상 후 nestin의 발현을 면역조직화학법으로 관찰하였다. 실험동
물은 성숙한 Sprague-Dawley 계통 흰쥐를 사용하였으며, 양쪽 콩팥동맥을 45분간 clamp로 결찰하여 허혈-재관
류 손상을 유도하였다. 대조군의 경우 동맥을 clamp로 잡지 않고 그대로 두었다. 재관류 후 1일, 3일, 5일, 7일,
14일의 콩팥을 periodate-lysine-paraformaldehyde 고정액으로 고정한 후 vibratome으로 50 μm 절편을 작성하였
고, nestin 항체를 사용하여 포매 전 면역조직화학법을 시행하였다. 정상군에서 nestin은 토리의 발세포에서 강하
게 발현되었다. 토리의 모세혈관을 제외한 거의 모든 모세혈관의 내피세포에서도 nestin 양성 반응이 관찰되었으
나 겉질에서는 발현정도가 약하고 속질에서는 양성반응이 증가하였다. 콩팥세관사이의 사이질세포에서도 양성
반응을 보였으나 속속질의 특이한 사이질세포인 지질함유사이질세포는 음성반응을 보였다. 허혈-재관류 후 전반
적으로 nestin의 발현이 1일군부터 7일군까지는 발현이 증가하였으나 14일군에는 발현이 감소하여 정상군의 반
응정도로 회복되었다. 그러나 발세포와 모세혈관 내피세포에서의 발현정도는 거의 변화가 없었다. 1일군부터 7
일군까지 nestin 발현의 증가는 사이질세포에서 양성반응세포의 수가 많아지고 반응성이 증가하였기 때문이다.
특히 정상군에서는 3일군으로부터 14일군까지 nestin 음성반응이었던 콩팥세관세포중 일부에서 양성반응이 관
찰되었으며, 이러한 세관주위의 사이질은 더욱 강한 양성반응을 나타내었다. 이상의 결과로 콩팥의 허혈-재관류
손상 시 nestin은 사이질세포에서 발현이 증가하며 재생 중의 콩팥세관세포에로 발현되는 것으로 보아, 콩팥에
서 epithelial-mesenchymal transformation에 관련된 표지자로 사용될 것으로 생각된다.Nestin, a type VI intermediate filament, is a marker for stem cells. Although it is expressed abundantly
in various organs during development, its expression in adults is restricted to certain types of cells. However, nestin
is reinduced in activated cells involved in the regeneration and survival of injured tissues. We investigated the expression
of nestin in the ischemia-reperfusion injured rat kidney by immunohistochemistry using anti-nestin antiserum.
Kidneys were preserved from normal adults and at 1, 3, 5, 7 and 14 d after ischemia-reperfusion injury, and processed
using pre-embedding. In the normal adult kidney, nestin was expressed strongly in the glomerular podocyte. Capillary
endothelial cells, except for those of the glomeruli, showed nestin positivity. Fibroblast-like interstitial cells were also
nestin positive except for lipid-laden interstitial cells of the inner medulla. After ischemia-reperfusion injury, the renal
expression of nestin increased progressively to 7 d and then returned almost to a normal level by 14 d. These changes
of nestin expression were attributed mainly to changes in the number and staining intensity of immunostained interstitial
cells. The podocytes and endothelial cells showed no change in immunoreactivity throughout any stage in the
experimental animals. Interestingly, nestin-positive tubular cells, which were nestin negative in normal kidney, were
observed from 3 to 14 d. Nestin immunostained cells were increased in the interstitium around these nestin-positive
tubules. These results suggest that nestin is induced in both interstitial cells and regenerating tubular cells and that it
can be used as a histological marker related to epithelial-mesenchymal transformation in the injured kidney.
