배경 및 목적:Steinless-steel 임플란트는 금속에 대한 과민반응 및 알러지 반응을 일으키고 임플란트 주위로 섬유증식성반응을 일으킨다. 이러한 반응은 관동맥스텐트삽입후에도 관찰된다. 스텐트 삽입환자에서 금속알러지는 스텐트재협착과 연관이 있다는 보고가 있었다. 따라서 본 연구자등은 새로운 기법의 탄화물로 제조된 스텐트가 금속알러지 발생과 재협착의 발생과 유관한지를 조사해 보고자 하였다.방 법:2002년 4월부터 2004년 6월까지 스텐트시술을 받은 128명(남자 100명, 평균나이 60±10세)의 환자를 대상으로 하였다. 대상환자의 임상진단은 급성심근경색 64명, 불안정형 협심증 24명, 안정혐협심증 40명이었다. 대상환자는 무작위배정하에 기존의 Arthos stent(대조군) 또는 Arthos Inert Stent(연구군)를 시술하였으며 6개월 추적관동맥검사를 시행하였다. 시술후 3개월째 망간, 몰리브덴, 니켈 및 포타슘 디크로메이트(Chemotechnique diagnostics, Malmo, Sweden)를 사용하여 피부반응검사(첩포검사)를 시행하여 양성유무를 판정하였다.
결 과:
치료군과 대조군의 연령, 성별 및 위험인자 및 다혈관질환의 빈도는 양군간에 차이가 없었다(p>0.05). 치료군과 대조군의 정량적 관동맥조영술상의 비교에 있어서 시술전, 후 및 추적관동맥조영술상 참고혈관크기, 최소내경 및 협착의 정도는 유의한 차이가 없었다(p>0.05). 추적관동맥조영술은 94명의 환자(73.4%)에서 이루어졌으며 조영술상의 재협착(>50% DS)은 치료군에서 10명(20.4%), 대조군에서 12명(26.7%)로 통계적 차이는 없었다(p>0.05). 피부반응검사의 결과: 피부반응검사를 시행한 사람은 57명으로 전체의 44.5%의 환자에서만 시행되었다. 양성율은 망간(33%), 크롬(28%), 니켈(21%), 몰리브덴(2%) 순이었으며 한개이상 양성을 보인경우는 전체의 51%였다. 또한 4가지 금속물질 중 1개만 양성을 보인경우가 30%, 2개 11%, 3개 9%, 4개 모두 양성을 보인 경우가 2%였다. Arthos Inert stent군에서 피부반응검사 양성율은 60.6%(20/33)로 기존의 Arthos stent군의 37.5%(9/24)에 비해 오히려 높은 경향을 보였으나 통계적인 차이는 없었다. 재협착군과 비재협착군간의 비교에 있어서도 피부반응검사 양성율은 각각 48.8%(20/41) 56.3%(9/16)으로 양군간의 유의한 차이는 없었다.
결 론:
새로운 기법(ion implantation)으로 제작된 Arthos inert stent는 기존의 기법(carbon coating)을 사용한 Arthos stent에 비해 금속알러지와 스텐트재협착을 유의하게 감소시키지 못하였다.Background and Objectives:Metal allergy has been reported to be related to stent restenosis. Therefore, we sought to investigate the incidence of metal allergy with new carbon stent with ion implantation technique and its relationship to restenosis. Subjects and Methods:Between April 2002 and June 2004, 128 patients (100 male, mean age 60±10 years) were included in this study. The clinical diagnoses of the study patients included 64 acute myocardial infarction, 24 unstable angina and 40 stable angina patients. Patients were randomly assigned to receive either an Arthos Inert Stent (the study group) or an Arthos stent (the control group), with follow-up angiography scheduled 6 months later. Three months after stent implantation, skin patch tests, for the detection of metal allergies, were performed with manganese, molybdenum, nickel and chromium. Results:There were no differences between the age, genders, risk factors and multi-vessel involvement between the two groups, as well as no differences in the angiographic parameters and restenosis (p>0.05). There were no differences in the positive rates of the skin tests between the Arthos stent and Arthos Inert stent groups or between the restenosis and no-restenosis groups (p>0.05). Conclusion:The new carbon stent, with ion implantation, did not reduce coronary restenosis. An allergic reaction was not related to the stent technology or stent restenosis.
