배경 및 목적 : 운동부하 심전도 검사 시 회복기 1분 및 2분 동안의 심박수 회복은 흉통 환자 및 무증상의 검사자에서 다른 운동부하 검사 지표들과 독립적으로 전체 사망률의 증가를 예측할 수 있는 예후 지표로 알려져 있다. 하지만 이러한 심박수 회복이 Duke treadmill score(DTS) 등 다른 지표와 같이 관상동맥 질환의 유무 및 다혈관 질환의 유무를 예측하는데 도움이 될 것인가에 대하여는 아직 잘 알려져 있지 않다.방 법 : 운동부하 심전도 검사와 관상동맥 조영술을 3개월 이내의 간격으로 시행 받은 294명의 환자들을 대상으로 이들 중 전형적인 허혈성 심전도 변화나 흉통으로 검사를 중단했거나 목표 심박수까지 검사를 완료하였고 성별, 연령, 흡연, 고지혈증, 고혈압, 당뇨, 비만 등 관상동맥 질환의 위험인자들과, 베타차단제, 칼슘길항제 등의 사용 여부에 유의한 차이가 없었던 78명(정상 관상동맥 39명, 유의한 협착을 보인 39명)의 환자를 대상으로 양 군 간에 운동부하 지표들을 비교분석 하였고, 다혈관 질환 환자(22명)와 단일 혈관 질환 환자(17명)간의 지표들의 차이도 비교 분석하였다.

운동부하 심전도 검사에서 회복기 1분과 2분의 심박수 차이는 DTS와 함께 관상동맥 질환의 유무 및 다혈관 질환의 예측인자로서 유용한 지표의 하나가 될 수 있을 것으로 사료되었다.Background and Objectives:The recent data has revealed that the first 1 minute and 2 minutes of heart rate recovery of patients who are being evaluated for chest pain and asymptomatic adults are associated with the overall mortality. However, the clinical significance of the heart rate recovery after exercise testing during the first 2 minutes for predicting the presence or severity of coronary artery disease and the correlation with the Duke treadmill score (DTS) have not been fully evaluated. Subjects and Methods:The study population included 39 patients with significant coronary artery stenosis and 39 subjects with normal coronary arteries. All of them underwent a tread-mill exercise testing prior to coronary angiography. The differences in the heart rate recovery and the DTS between the two groups were investigated. The heart rate recovery and DTS between the multivessel disease group and single vessel disease group were also investigated. Results:The heart rate recovery during the second minute (calculated by the heart rate at 1 minute-the heart rate at 2 minute) after exercise and the DTS were significantly lower in the coronary artery disease group. These parameters were also significantly lower in the multivessel disease group than those in the single vessel disease group. The heart rate recovery during the second minute and the DTS had positive linear correlation (R=0.281, p<0.05). Conclusion:In this study, the heart rate recovery during the second minute was considered to be an additional important variable to predict the presence and severity of coronary artery disease.
