목 적: 급성 척수 손상에서 polyethylene glycol (PEG)과 황산 마그네슘(MgSO4)을 병합 투여하여 신경 보호 효과가 있는 지 알아보고자 하였다.
대상 및 방법: 흰쥐 수컷 Sprague Dawley 20마리(300-350 gm)를 사용하여 복강 내 마취 하에서 제 9-10 흉추 사이에 후궁 절제술을 한 후 Ohio State University (OSU) impactor를 이용하여 척수 손상을 시킨 후, 무작위로 10마리씩 2군으로 나누고 제1군은 PEG (1 g/kg)+MgSO4 (300 mg/kg)을, 제2군은 대조군으로 생리식염수(2ml)를 쥐의 경정맥을 통하여 수상 후 2시간, 그 후로는 매 6시간 마다 동일 용량을 총 5회 주입하였다. 행동평가는 수술 후 각각 2, 4, 7일 및 매주에 Basso, Bresnahan, and Beattie (BBB) score와 BBB subscore를 이용하였다. 감각 평가는 Von Frey hair monofilament를 이용하였고, Catwalk 프로그램을 이용한 보행분석을 시행하였으며 수상 후 6주째 결과와 수상 전의 검사결과를 비교하였다. 조직학적 평가는 수상 후 6주째 쥐를 희생시켜 척수 조직을 채취하여 Eriochrome Cyanine 염색으로 회색질과 백색질의 면적을 구하여 보존된 척수 조직을 정량화 하고 두 군에서 얼마나 척수 조직이 보존되었나를 알아보았다.
결 과: 6주째 평균 BBB score는 PEG+MgSO4 군 13.3±0.3, 생리식염수 군 11.4±0.2이었고, BBB subscore는 각각 9.1±1.1, 4.4±1.2이었다(p<0.05). 6주말에 시행한 감각 검사 및 보행 분석에서 두 군 간의 유의한 차이는 없었다. 조직학적 평가 결과 회색질의 면적은 두 군 간에 차이가 없었으나 백색질의 면적은 수상 중심부에서 각각 0.68±0.2, 0.41±0.04 mm2으로 PEG+MgSO4 군에서 유의 하게 백색질의 보존이 많았다(p<0.05).
결 론: 급성 척수 손상 후 PEG와 황산 마그네슘의 병합투여는 운동 능력의 향상과 손상된 척수 신경의 보존이 확인되어 신경보호 효과가 있는 것으로 판단된다.Purpose: To evaluate the neuroprotective effect of combination therapy of polyethylene glycol (PEG) and magnesium sulfate (MgSO4) after a spinal cord injury.
Materials and Methods: Twenty Sprague Dawley male rats (300-350 gm) had a spinal cord injury after T9/10 laminectomy using an Ohio State University (OSU) impactor under intraperitoneal anesthesia. The animals were randomized to receive either PEG (1 g/kg)+MgSO4 (300 mg/kg) or saline (2 ml) via carotid vein after 2 hours of injury and then every 6 hours for 5 times. The behavioral outcome assessments were performed on days 2, 4 and 7, and then every week using the Basso, Bresnahan, and Beattie(BBB) score and subscore. The animals also underwent sensory threshold testing using a von Frey monofilament device and gait analysis with Catwalk program before and 6 weeks after cord injury. The animals were sacrificed at the end of 6 weeks and histologic assessment was performed to measure
the areas of white and gray matter.
Results: For the animals treated with PEG+MgSO4 and saline, the mean BBB scores at 6 weeks post-injury were 13.3±0.3, 11.4±0.2 and the BBB subscores were 9.1±1.1, 4.4±1.2 respectively (p<0.05). No significant differences were found in sensory testing and gait analysis between the two groups. Histologic assessment revealed no significant difference in gray matter sparing but the areas of white matter at the lesion epicenter were 0.68±0.2, 0.41±0.04 mm2 in the PEG+MgSO4 and saline groups respectively, which indicated significant sparing of white matter in PEG+MgSO4 group (p<0.05).
Conclusion: The combination therapy of polyethylene glycol and magnesium sulfate improved the motor function and showed significant histological sparing of the spinal cord after an acute spinal cord injury in rats.
