본 연구의 목적은 2004년부터 시행되고 있는 의료기관평가제도의 문제점을 메타평가(meta-evaluation)의 관점에서 평가하고, 향후 제도적 개선책을 제시하는데 있다. 메타평가를 위해 본 연구에서는 평가환경, 평가투입, 평가과정, 평가산출, 평가환류 등 5가지 요소를 중심으로 연구모형으로 구성하였다. 분석결과 평가환경에서 평가목표의 명료성이 인정되지만 평가정향에서 정부는 규제, 의료기관은 자율성을 강조하는 것으로 나타났다. 평가투입 측면에서 볼 때 평가조직의 체계성과 독립성, 평가인력의 전문성? 공정성? 적절성, 평가예산의 충분성 측면에서 문제점이 있는 것으로 나타났다. 평가과정에서는 지표의 과다와 복잡성 문제, 단기 또는 시설 중심의 평가지표 편향성 문제, 평가의 중복성 등이 지적되었다. 평가산출과 관련해서 의료기관의 낮은 수용성, 일반인의 활용 가능성 등이 문제로 분석되었으며, 평가환류와 관련해서는 결과의 활용정도가 낮다는 점, 관련 인센티브가 부족하다는 점이 지적되었다. 이와 같은 분석을 토대로 본 연구에서는 의료기관평가제도의 발전을 위한 향후 정책적 제언을 하고 있다.This study aims to analyze the Hospital Quality Assessment, which have implemented by Korea government since 2004, through using the metaevaluation framework. Evaluation standard for metaevaluation consists of five factors, such as environment, input, process, output and feedback. As a result of the meta-evaluation, first, there are conflicts around the basic orientation of evaluation between evaluator and evaluatee. From input sides, there are lacks of systemic construction, independence in evaluation organization, of expertise, fairness and appropriateness in evaluation staffs and of sufficiency in budget. In evaluation process, there are too many or complex evaluation indicators, heavily focusing on quantitative or physical sides. In output side, both lower acceptance and utilization of evaluation information are noticed without demanding incentive system.
