초등학교 3학년 아동을 대상으로 아토피 피부염 예방과관리를 위한 영양교육을 실시하였다. 3학년 총 6개 반 중 3개반은 영양교육군(교육군), 나머지 3개 반은 비교육군(대조군)으로 선정하여 전체를 대상으로 영양교육(전) 설문조사를 실시하고, 실험군 97명을 대상으로 약 6주 동안 영양교육을 실시한 후 3학년 전체를 대상으로 영양교육(후) 설문조사를 실시하고, 총 191명을 대상으로 분석을 실시하여 영양교육 효과를 평가하였다. 본 연구의 목적은 아토피 예방 및 관리를위한 영양교육을 통하여 잘못된 식습관을 교정하고, 올바른식습관을 형성을 토대로 식행동 변화를 도모하여 궁극적으로 아토피를 이기는 식생활을 실천할 수 있도록 하고자 하는것으로, 아토피 피부염을 예방하고 관리하기 위한 방안으로써 영양교육의 필요성을 제시하고자 한다. 영양교육은 총 6차시로 진행되었으며, 각 차시별 교육내용을 살펴보면 1․2차시는 식품첨가물의 유해성을, 3차시는 설탕(당)의 유해성을, 4차시는 아토피와 편식과의 관계를 이해하고, 5차시는 아토피 관련 식생활 정보를 습득하고, 6차시에는 제철(교육 당시 7월) 과일을 이용하여 오미자 화채를 만드는 실습을 하였다. 식생활 태도 조사 결과, 영양교육 전․후에 따라 대조군의경우는 식생활 태도 점수가 평균 3.98(SD=0.59)에서 사후 평균 3.99(SD=0.71)점으로 나타나 변화를 보이지 않았으나, 교육군에서는 아토피 피부염 관련 영양교육 후 식생활 태도 점수가 평균 3.84(SD=0.59)에서 평균 4.16(SD=0.65)으로 유의적으로 증가한 것으로 조사되었다. 영양교육 프로그램 실시전․후에 두 집단 사이 그리고 집단 내에 유의한 차이가 있는지를 조사하기 위해 각각 paired t-test를 실시한 결과, 대조군의 경우 전․후 차이가 없었고, 교육군의 경우는 전․후 유의한 차이가 있는 것으로 나타났다. 간식행동(간식 먹는 횟수,섭취 이유, 간식 종류, 간식 섭취 장소, 간식 구매 시 고려사항)에 대해 조사한 결과, 아토피 피부염 관련 교육을 받은 교육군의 경우, 간식 종류로 집에서 만들어 주는 간식이 40.2%에서 57.7%로 17.5%가 증가하였고, 간식을 섭취하는 장소가집으로 응답한 비율이 68%에서 81.4%로 13.4%가 증가하였다. 주변동질성검정을 통하여 대조군과 교육군의 각 항목별아토피 피부염 관련 영양교육 프로그램 적용 후 사전-사후검사에 대한 차이를 살펴본 결과, 교육군의 경우 교육군에서아이스크림, 빵류, 초콜릿류 등에서 통계적으로 유의하게 섭취 빈도가 감소함을 보였으며(p<0.01), 스넥류, 사탕 & 캐러멜류, 스포츠 & 이온음료류도 통계적으로 유의하게 섭취 빈도가 감소함을 보였다(p<0.05). 대조군과 교육군의 간식 섭취 빈도에 관한 사전 조사 결과는 통계적으로 유의한 차이를 보이지 않았으나, 교육군에게영양교육을 적용 후 사후 검사를 한 결과에서는 초콜릿류의간식 섭취 빈도가 통계적으로 유의하게 감소한 것을 알 수있었다. 아토피 피부염 관련 영양교육 프로그램을 적용한 결과, 교육군에서 간식 종류에 따라 선호도의 유의적인 감소를 보였다. 즉, 튀김류, 스넥류, 초콜릿류에서 통계적으로 유의한 선호도의 감소를 보였으며(p<0.05), 빵의 경우도 통계적으로 유의한 선호도가 감소된 것을 알 수 있었다(p<0.01). 이상의 연구는 아토피 피부염 관련 영양교육 프로그램을적용하여 사전-사후 검사를 한 결과, 전반적인 식생활 태도는물론 아토피 피부염에 직접적으로 관련이 있는 간식행동(간식 섭취 장소, 간식 종류, 간식 섭취 빈도)과 간식의 종류별에따른 선호도에 유의적인 변화를 보였다. 즉, 아토피 피부염의 예방 및 관리에 도움이 될 수 있도록 긍정적인 방향으로개선되었음을 알 수 있었다. 그러므로 아토피 피부염 예방과관리를 위한 영양교육의 효과가 있는 것으로 판단되며, 보다실효성 있는 교육 효과를 위해서는 교육기회의 확대와 아토피 관련 정책 연구 시 교육현장에서 활용할 수 있는 다양한영양교육 자료 개발이 요구된다.Atopic related skin symptoms must be controlled continually. Because it is involved with food, eating behaviors play an important role in the prevention and management of atopic dermatitis. The aim of this study was to develop and apply nutrition education program for the prevention and management of atopic dermatitis on elementary school students. After applying the nutrition education program, our research attempts to evaluate the effectiveness of such nutrition education program. This study examined the effects of a nutrition education program on atopic dermatitis-related snacking behaviors, eating attitudes, and food behaviors. Subjects were 3rd grade elementary school students in Daejeon. A 6-week nutrition education program was implemented to 97 children as the educated group and another 94 children were included in the study as the control group. A self-administered questionnaire was used to assess the effects of nutrition education program. As results, eating attitudes reached significant improvement by the nutrition education program(p<0.05). In a paired t-test, the control group did not show significant difference, but the educated group showed a significant difference. In snacking behaviors, the two groups did not show statistical difference except for some considering factors. Also, there were no significant differences of the two groups after nutrition education program, except the snacking place after the nutrition education group. However, in the educated group, the types of snacks made by mothers increased 17.5% from 40.2% to 57.7%, and in-house as snacking place increased 13.4% from 68% to 81.4%. In the results of before and after the nutrition education program, the program showed some effect on the prevention and management of atopic dermatitis. Therefore, these results showed foundation for nutritional education and various educational programs are needed afterward to increase the effect of nutrition education in the prevention and management policy related to atopic dermatitis.
