목적: 알코올 과다 섭취력이 없는 건강검진자 중 지방간 초음파 소견을 보이나 정상 간기능 수치를 보이는 무증상 환자를 대상으로 임상특징과 대사증후군과의 관련성에 대해 알아보고자 하였다. 대상 및 방법: 포천중문의대 분당차병원 건강검진센터에서 건강검진을 시행 받은 30세 이상의 성인 중 연구 동의서에 서명한 538명을 대상으로 하였다. 이 중 HBsAg 양성자, anti-HCV 양성자 알코올 과다 섭취력이 있거나 알코올 섭취력을 알 수 없었던 자, 경구용 혈당강하제를 복용 중이거나 인슐린 치료를 받고 있는 당뇨병 환자를 제외하였다. 신체 계측, 혈액 검사, 복부초음파검사를 실시하였으며, 대사증후군은 The National Cholesterol Education Program Adult Treatment Panel III (ATPIII)와 아시아 태평양 비만 기준에 따라 정의하였다. 간기능 검사 결과에 따라 정상 ALT 수치를 보이는 대상자만을 모아 초음파상 지방간 유무에 따라 임상양상과 대사질환의 유병률을 비교하였으며, 이후 이들을 다시 ALT 수치에 따라 세분하여 비교하였다. 결과: 나이, 체질량 지수 허리둘레 엉덩이 둘레, 혈압, 간기능 검사(AST, ALT, γ-GT), 혈압, 백혈구 수, 요산, 총 콜레스테롤, 중성지방, 저밀도 지단백 콜레스테롤, 공복인슐린, HOMAℝ, 베타세포기능은 지방간 초음파 소견군에서 의미 있게 높았다. 지방간 초음파 소견을 보이는 경우 정상 간기능 검사를 보이더라도 대사증후군과 인슐린 저항성의 유병율이 높았으며 교차비가 각각 4.53 (95%CI; 2.35-8.75), 4.83 (95%CI; 2.71-8.59)이었다. 정상 초음파 소견군과 지방간 초음파 소견군을 이분위수에 따라 낮은 정상 ALT군과 높은 정상 ALT 군으로 나누어 분석한 결과, 양 군 모두에서 높은 정상 ALT 군에 속하는 경우 대사증후군을 비롯한 대사 이상 질환의 유병률이 유의하게 높았다(P<0.01). 비알코올지방간 진단에 가장 예민도와 특이도가 높은 ALT수치는 18.5IU/L 였다(area under curve=0.746, p <0.01). 결론: 알코올 과다 섭취력이 없는 환자에서 지방간 초음파 소견을 보이는 경우 정상 간기능 검사를 보이더라도 대사증후군과 대사이상 질환의 유병률이 높았다. 또한, ALT가 정상 범위이더라도 높은 수치를 보이는 경우 대사증후군과 다른 대사 이상 질환들의 유병률이 높았다. 지방간과 대사증후군의 선별을 위하여 앞으로 대규모 전향 연구를 통하여 정상 ALT 수치의 범위에 대해 재고가 필요하다.Background/Aims: Nonalcoholic fatty liver disease (NAFLD) is known to be closely associated with various metabolic abnormalities including metabolic syndrome. However, there are few data available on the association of metabolic syndrome with the sonographically fatty liver and normal range of liver function test. The purposes of this study were to find the incidence of ultrasonographic fatty liver with normal range of liver function test and to evaluate the association with metabolic syndrome in apparently healthy Korean adults. Methods: We examined 538 men and women, aged 30-80 years, who participated in a health screening test. Among the people with normal ALT level, we compared clinical characteristics and prevalence of metabolic disorders according to the presence of nonalcoholic sonographyally fatty liver, and then they were subdivided into upper normal range and lower normal range of ALT level. Results: Compared to the people without sonographic fatty liver, people with sonographic fatty liver and normal range of ALT level had odds ratios for metabolic syndrome of 4.53, insulin resistance 4.83, hypertension 2.69, dyslipidemia 6.90, and obesity 5.39, respectively. Furthermore, the prevalence of metabolic syndromes and other metabolic disorders were increased in both sonographically fatty liver group or ultrasonographically normal liver group with upper normal range of ALT level compared with lower normal ALT level (p<0.01). Conclusions: The nonalcoholic sonographically fatty liver was strongly associated with metabolic syndrome and common metabolic abnormalities even with normal liver function test.
