이 연구는 정체성을 의사소통적 재구성체로 규정하고 언어와 의사소통 속에서 탐구하는 자리매김(positioning)의 이론과 방법론을 토대로 의사와의 의사소통에 대한 체험 인터뷰에서 간호사가 자리매김한 의사의 정체성을 살펴보는 데 목적이 있다.
의사와의 의사소통에 대한 체험 인터뷰에서 간호사가 자리매김한 의사의 정체성은 부분적으로 긍정적인 측면도 있었으나 전체적으로는 부정적인 것으로 나타났다. 권위적이고 우월적인 행동 성향을 지닌 사람(1), 개인적 자기 중심적 행동 성향을 지닌 사람(2), 무례하고 몰염치한 행동 성향을 지닌 사람(3), 불공정하고 비일관적인 행동 성향을 지닌 사람(4), 불성실하고 무성의한 행동 성향을 지닌 사람(5), 공격적인 행동 성향을 지닌 사람(6) 등의 6개 범주로 재구성된 의사의 부정적 정체성의 본질적인 특징은 인간으로서 간호사의 존엄성 그리고 간호사의 직역과 업무 수행의 의미에 대한 가치를 인정하지 않고 존중하지 않는 의사의 가치관과 그에 따른 행동과 태도가 있는 것으로 보인다. 의사의 타인에 대한 비존중의 바탕에는 사람을 가치 판단의 본질적 기준으로 삼지 않고 사람이 가지고 있는 다양한 외적 조건(예: 학력, 외모, 경제력, 직업 등)을 기준으로 삼는 비인본적 세계관, 그러한 외적 조건에 따라서 사람을 계급적으로 구분하고 차별하는 위계적 세계관, 그리고 타인을 존중하고 배려하지 않는 개인적이고 이기적인 성향이 있는 것으로 판단할 수 있다. 이렇게 형성된 의사의 가치관과 세계관으로 인해서 의사는 간호사를 존중하지 않아도 되는 사람이고 경우에 따라서는 함부로 대해도 되는 사람이자 직역으로서 자리매김하는 것으로 보인다.
의사소통 능력을 본질적으로 개선하기 위해서는 의사소통의 분석을 통해서 밝혀진 타인에 대한 화자의 문제적 태도와 가치관을 변화시키고 개선하는 것이 중요하다. 그러므로 의사소통 교육은 의사소통의 분석을 통해서 밝혀진 화자의 태도와 가치관을 통해서 개선시키고 이를 통해서 결과적으로 의사소통이 개선되는 방식으로 이루어져야 할 것이다. 비존중적인 행동과 태도로 인해서 발생하는 상호 무시와 멸시의 악순환에서 벗어나고 의사와 간호사 모두의 자존감과 행복감을 제고하기 위해서는 의사소통 능력의 개선이 필요하다. 의사소통 능력의 개선은 의료인의 태도와 가치관의 변화와 개선을 위한 교육이 먼저 선행되어야 한다. 이를 위해서 특히 의료인이 비존중적이고 무례하며 공격적이고 비공감적인 행동 성향을 갖게 된 이유와 과정이 무엇인가를 밝혀내는 것도 필요하다. 이에 대한 심도있는 논의는 향후 연구의 과제로 남긴다.The purpose of this article is to investigate nurses positioning act with regard to the physicians. This research bases on 10 expert interviews with nurses about their experiences of communication with physicians and the positioning theory. This study tries to answer the following questions: how do the nurses position the physicians; how do the physician position the nurses in the nurses' notion?; what is important and valuable in the relation between nurses and physicians?; as whom do nurses want to be positioned and recognized by physicians? After introducing the positioning theory and term of position the mutual positioning acts of the nurses and physicians are analysed. According to the analysis the physicians are authoritative, self-oriented and selfish, impolite and shameless, unfair and inconsistent, half-hearted and careless, emotional and aggressive. These results implicate the following self positioning of the nurses: the nurse has the same dignity as doctor; the nurse performs independent and essential work for the treatment of a patient's illness; the nurse deserves the warm consideration and kindness of physicians; the nurse has to be treated with courtesy; the nurse is entitled to fair and equal rights; the nurse needs to be treated sincerely and seriously by physicians; the nurse needs to be treated kindly and polite by physicians.
