본 연구는 인천지역의 응급의료상황을 파악하기 위하여 지난 3년간(2014-2016) 인천지역 20개 응급의료기관에 내원한 응급실 이용자를 대상으로 응급실 사망률에 미친 요인의 특성을 일반적 요인, 병원단계 요인, 환경적 요인, 그리고 거버넌스 요인으로 나누고 관련 변수들과의 관계를 GEE분석방법을 이용하여 분석하였다. 분석결과는 첫째, 그동안 사망률과 관련한 이론적 논의를 확인할 수 있었던 측면에서 학술적 의미가 있다. 환자의 연령에 따라 사망률의 차이가 있었으며, 여성환자 비율과 야간이용자 비율이 높을수록 사망률이 증가하였다. 병원 단계별, 환경적 요인에 따라 사망률의 차이가 있었다. 둘째, 정책적 측면에서는 인천지역에 국한하여 그 의미를 분석하였다는 점에서 정책적 함의를 갖는다. 전문응급의료센터 지정수가 증가할수록 사망률이 감소하였으며, 의료기관 수 증가, 평균 응급의료 전문의 수 증가 등이 응급실 사망률을 감소시키는 요인으로 분석되었다. 또한 인천지역의 생활권, 병원 단위, 의료자원, 환자주소지 등에 따라 인천지역의 군·구 간 차이가 상당한 것으로 나타났다. 이러한 분석결과는 향후 지역내 응급의료체계를 구축할 때, 생활권에 맞는 권역응급의료센터 추가지정, 지역응급의료센터급의 배후진료 기능 강화, 권역심뇌혈관 질환센터와 연계한 응급의료체계 구축, 18세 미만에 대한 외상체계 구축, 권역외상센터 기능강화 및 지속적인 정책의제화 등 최근 인천지역내에서 응급의료체계를 개선하려는 다양한 논의에 이론적 기초자료로 활용될 수 있을 것으로 기대된다.
The purpose of this study is to analyze the mortality rate of emergency rooms in Incheon Metropolitan City to examine the factors affecting it. Three year data(2014-2016) were gathered from National Emergency Medical Information Network and other resources. The statistical analysis to identify the factors affecting mortality was performed using GEE analysis. As a result, there were differences in the mortality rate according to the proportion of patients older than 65 years and under 18 years, female patients and of nighttime users. Considering environmental and policy factors as well, there were differences according to selected variables such as the number of medical institutions and emergency medical specialists. In addition, there are considerable differences in the results of emergency medical treatment according to area, hospital unit, and medical resources. An increase in the number of medical institutions and an increase. A complete regional emergency medical system requires a designated regional emergency medical center appropriate for the living zone, strengthened rear care functioning for the regional emergency medical center, connection with a cardiovascular disease center, and a trauma system for minors under 18 years of age within a system of local governance. The policy implications of this study follow that it can contribute to a reduction in the mortality rate of emergency rooms by establishing a community-based emergency medical care system through community governance.
