본 연구는 노인의료복지시설의 지역별 수요 대비 공급의 적정성을 분석하여 노인의료복지시설 분포의 문제점을 파악하고, 노인의료복지시설 이용자의 권리 차원에서 시설 이용 기회의 균등성 보장을 위한 정책적 대응 방안을 모색하는 것을 목적으로 하였다. 이를 위해 수요와 공급의 시나리오별로 전국 노인의료복지시설의 현황을 파악한 후, 시·군·구별 수요 대비 공급의 적정성을 ① 형평성, ② 공간상관성 분석, ③ 충족도를 활용하여 검토하였다. 자료 분석 시 행정안전부의주민등록인구 자료와 국민건강보험공단의 등급 판정 현황 자료 및 장기요양기관 현황 자료를 활용하였으며, 형평성과충족도는 주어진 계산식을 통해 도출하고, 공간상관성 분석 및 공간분석은 GeoDa와 QGIS 3.16 프로그램을 활용하였다.
연구 결과 첫째, 도 지역에 비해 광역시의 비형평 수준이 전반적으로 높았으며, 광역시도 내 중심도시와 도서산간지역은대부분 노인의료복지시설의 공급이 부족한 반면 중심도시의 외곽지역은 공급이 과잉된 특성을 보였다. 둘째, 세 가지의모든 시나리오에서 노인의료복지시설의 정원 1인 대비 수요의 분포는 공간적 자기상관성을 갖는 것으로 나타났다. 셋째, 노인의료복지시설의 충족도는 전반적으로 과잉공급된 특성을 보였으나, 지역 간에 큰 차이가 있음이 확인되었다. 이러한결과는 향후 노인의료복지시설의 수요에 대응하는 공급량 조정에 있어 지역별 수요-공급 분포를 적절히 고려해야 하며, 특히 시·구 지역과 군 지역의 특성을 반영하여 장기적 수급 계획을 마련할 필요가 있음을 시사한다.
This study focused on the adequacy of supply and demand for long-term care facilities in order to identify problems of facility distribution and suggest policy improvements which are guaranteeing equality of users’ opportunity. The adequacy of supply and demand for the facilities was drawn from the analysis of equity, spatial correlation, and occupancy rates, based on the combination of several supply and demand scenarios. The study obtained ‘population statistics based on resident registration’ data from Ministry of the Interior and Safety(MOIS) and ‘State of Long-Term Care Institutions and decisions on Long-Term Care Grade’ data from National Health Insurance Service(NHIS) and the collected data was analyzed using calculated formulas, GeoDa, and QGIS 3.16 program. According to the results of this study, first, inequity level was generally high at the metropolitan cities than the provinces and the center area of the metropolitan cities and provinces showed undersupply whereas the suburb area showed oversupply. Second, there were spatial correlations in every scenario as to the distribution of ‘demand per facility’s capacity’. Third, while the facilities’ occupancy rates presented overall oversupply, there were large differences among regions at the municipal level. These results imply that the regional differences of current supply and demand distribution should be properly considered in the adjustment process of future supply to demand.
