연구배경: 급속한 인구고령화에 따른 치매 유병률 증가는국외는 물론 국내에서도 주요 공중보건 이슈로서, 인지기능저하를 조기에 발견하는 것은 개인은 물론 국가의 의료비부담 감소와 환자와 가족의 삶의 질 향상에 중요하다. 따라서 본 연구에서는 50세 이상 성인을 대상으로 치매선별검사행위 영향 요인을 파악하여 효과적인 치매 관리를 위한 중장기 전략 개발에 기초 자료를 제공하고자 시도되었다.
방법: 2018년도 지역사회건강조사 자료를 활용하여 치매선별검사 수진 항목에 응답한 지역사회에 거주하는 성인108,500명을 대상으로 표출하였다. 표출된 자료는 IBM SPSS version 26.0를 이용하여 기술통계, 복합표본 교차분석, 복합표본 로지스틱 회귀분석을 이용하여 분석하였다.
결과: 연구 대상자의 13.4%만이 치매선별검사행위를 한반면 인지장애 경험이 있다고 응답한 경우는 49.4%로 높게나타났다. 치매선별검사 참여율을 높이는 영향 요인으로는고령, 여성, 직업이 없는 경우, 낮은 교육수준과 소득수준, 농촌지역에 거주, 고혈압을 진단받은 경우, 인지장애 경험을 한 경우, 치매 상담콜센터를 인지하고 있는 경우가 제시되었다.
결론: 본 연구를 통해 치매선별검사 참여율이 낮은 대상자들을 고려하여 치매선별검사를 강화시킬 수 있는 국가적차원의 접근이 요구된다. 특히 주관적 인지장애 경험이 있는 대상자에게 치매를 예방할 수 있는 프로그램 개발과 증상조절 방안 모색이 필요하고 치매 상담콜센터와 같은 기관에 대한 적극적인 홍보가 요구된다.
Background: This study aimed to identify factors influencing dementia-screening behaviors among community-dwelling middle-aged and older adults using the Korean Community Health Survey data in 2018.
Methods: This cross-sectional study included community-dwelling adults aged ≥50 years. We used data related to socio-demographic and health-related factors and subjective cognitive function. We conducted a chi-squared test and multiple logistic regression analysis with weighted analysis.
Results: Of 108,050 participants (men, 46.6%; ≥65 years old, 41.5%), only 13.4% (20,404 participants) performed dementia-screening. Using multiple logistic regression, we found that advanced age, female, and unemployed, having a low level of education, low monthly family income, and rural residence, no alcohol intake, no-smoking, hypertension, cognitive dysfunction, and awareness of the dementia helpline service center were positively associated with dementia-screening behaviors.
Conclusions: This study suggests the application of holistic approaches to improve dementia-screening behaviors among middle-aged and older adults considering their socioeconomic and health related conditions. Our findings highlight that government health programs should consider socio-economic status and awareness of dementia helpline services in designing dementia prevention programs.
