정신건강복지법 제정 이후 우리나라 정신건강 정책의 기본 방향은 전 국민을 대상으로 하는 정신질환의 사전예방 및 치료부터 정신질환자와 그 가족을 위한 재활 및 회복 지원에 이르기까지 국가 주도의 책임 있는 환경조성을 강조하고 있다. 이러한 환경조성에 무엇보다 중요한 것은 각 지역의 특성에 맞는 서비스 개발 및 서비스 제공 모델을 구축하는 것이다. 본 연구는 인천광역시 지역 특성에 맞는 정신건강 서비스 개발 및 전달체계 구축 방안의 기초를 마련하기 위하여, 인천광역시 거주 주민들의 정신건강 격차가 존재하는지 살펴보고자 수행되었다.
본 연구의 분석에는 지역사회건강조사와 인천광역시 정신건강 통계가 사용되었다. 연구 수행 당시를 기준으로 가장 최신자료인 2021년 지역사회건강조사 자료를 활용하였으며, 전체 응답자 중 인천에 거주하는 8,985명의 응답을 분석하였다. 종속변수인 인천광역시 주민들의 정신건강 수준은 스트레스 인지, 우울감 및 자살 생각 경험, 고위험 음주의 네 가지로 나누어 살펴보았고, 모두 이분형 변수로 측정하였다. 인천광역시 주민들의 정신건강 격차는 인구사회학적 요인, 경제적 요인, 건강 관련 요인, 지역적 요인을 중심으로 살펴보았으며, 빈도분석, 카이자승검정, 다층 로지스틱 회귀분석을 수행하였다.
본 연구의 주요 결과는 다음과 같다. 첫째, 인천광역시 주민들의 스트레스 인지, 우울 및 자살 생각 경험, 고위험 음주 여부에 인구사회학적 요인, 경제적 요인, 건강 관련 요인과 같은 개인적 특성에 따른 차이가 존재하였다. 둘째, 군구별 정신건강 격차 분석결과, 지역에 따른 다소간의 정신건강 지표상의 차이가 발견되었다. 셋째, 지역별 정신건강 격차의 잠재적 이유를 탐색하기 위해 각 군구의 정신건강 예산 및 인력이 주민들의 스트레스 인지, 우울감 및 자살생각 경험, 고위험 음주와 관련이 있는지 살펴본 결과, 1인당 정신건강 예산이 많을수록 지역주민의 스트레스 인지, 우울감, 자살 생각 경험 가능성이 통계적으로 유의미한 수준으로 낮게 나타났다.
본 연구는 인천광역시 주민을 대상으로 정신건강 격차가 존재하는지 개인 및 지역적 요인을 종합적으로 고려한 연구라는 점에서 의의가 있다. 지금까지는 지역의 인구 현황, 정신건강 지표 등 지역적 정신건강 욕구에 대한 고려 없이 예산이 배분되고 사업이 수행되는 경향이 있었다. 본 연구는 인천광역시 주민들의 개인 및 지역별 정신건강 격차에 대한 분석을 바탕으로 근거기반의 정책 방안을 제안하였다.
Since the enactment of the Mental Health Act, the goal of Korea's mental health policy has been creating a state-led environment, covering from the prevention and early detection of mental illnesses for the general population to rehabilitation and recovery for people with psychiatric disabilities and their families. The most crucial thing in creating such environment is developing a mental health care system tailored to the needs of each region. This study aimed to examine whether there are mental health disparities among residents of Incheon Metropolitan City to guide policy efforts in developing a mental health service system in Incheon Metropolitan City.
The data for the current study were obtained by combining the Community Health Survey and public data. From the 2021 Community Health Survey, the most recent data available at the time of the study, 8,985 respondents living in Incheon Metropolitan City were analyzed. The mental health level of Incheon Metropolitan City residents was measured into four categories: perceived stress, depression and suicidal thoughts, and high-risk drinking, all measured as binary variables. The mental health disparities among Incheon Metropolitan City residents were examined, focusing on demographic, economic, health-related, and regional factors. Frequency analysis, chi-square test, and multi-level logistic regression analysis were performed using Stata SE 13.
The findings of this study are as follows. First, there were differences in the perception of stress, the experience of depression, suicidal thoughts, and high-risk drinking by demographic, economic, and health-related factors. Second, such differences were also found by county. Third, each county's mental health budget was related to residents' stress perception, depression, and suicidal thoughts, but not to high-risk drinking.
Until now, budgets have tended to be allocated, and programs promoting mental health among residents tended to be carried out without considering local mental health needs. This study discussed the need for an evidence-based policy to address mental health disparities in Incheon Metropolitan City.
