본 연구는 외국인 노인장기요양보험의 이용 특성 분석을 목적으로 한다. 다양한 외국인 인구집단이 국내 이주하며 외국인 연령 구성에 변화가 발생하였고, 큰 폭으로 고령 외국인 인구 비율이 높아지고 있다. 고령 외국인 증가로 노인장기요양보험 수요가 높아지고 있지만, 외국인 노인장기요양보험에 대한 연구 담론 및 분석이 부재하였다. 이에 본 연구에서는 외국인 노인장기요양보험 급여이용 특성을 분석하고자 하였다. 본 연구는 내국인-외국인 인정자의 급여이용 평균을 비교하고, 외국인 인정신청 기간에 따른 급여이용 특성을 분석하였다.

분석 결과, 외국인 인정자는 2017년 대비 2021년 52.9% 증가하여, 외국인 장기요양 수요가 크게 증가하고 있었다. 전체 급여 이용에 있어 내국인 급여이용 총액이 높았고, 재가급여 및 복지용구 이용에서도 동일한 추세를 보였다. 하지만, 외국인 인정자의 시설급여 평균은 내국인보다 높게 나타났다. 외국인 건강보험 자격 취득 후 장기요양 인정신청 기간(duration until application for LTC after NHIS enrollment) 측정 결과, 건강보험 자격 취득 후 곧바로 장기요양 인정신청을 한 외국인이 가장 높았고, 인정신청 기간이 길어질수록 인정자 수 또한 감소하였다. 자격 취득 후 인정신청 기간을 6구간으로 나눠 분석한 결과, 전체 급여는 단기/장기 기간 인정신청자의 급여이용액이 높았던 반면, 시설급여는 인정신청 기간이 짧을수록 급여이용이 높았다.

The purpose of the study is to analyze the utilization of long-term care insurance by foreigners. As various foreign population groups migrated to Korea, changes occurred in the age composition of foreigners, and the proportion of the elderly foreign population is significantly increasing. The demand for long-term care insurance has increased as the number of elderly foreigners has increased, but there is a lack of research and analysis on long-term care insurance. Therefore, this study aimed to analyze the characteristics of foreigners’ long-term care insurance use. This study compared the average amount of use between nationals and foreigners and analyzed the characteristics of use according to the duration between enrollment in the National Health Insurance Service and application for long-term care.

Our analysis found that the number of recognized foreigners increased by 52.9% in 2021 compared to 2017, indicating that the demand for long-term care for foreigners significantly increased. The total amount of use by Koreans was higher than that by foreigners, and the same trend was shown in the use of home-based benefits and equipment service. However, the average institution-based benefit for foreigners was higher than that for Koreans. When examined in terms of the duration until application for LTC after NHIS enrollment, the highest percentage of foreigners applied for long-term care immediately after enrollment in the National Health Insurance. Our analysis of the duration until application, divided into 6 sections, showed that the total benefit use was high for short- and long-term applicants. On the other hand, for institution-based benefits, the shorter the duration until application for LTC, the higher the use of benefits.
