본 연구의 목적은 우울장애의 자살위험 요인을 확인하여 우울장애 진단을 받은 사람의 자살을 예방할 수 있는 방안을 제안하는 것이다. 본 연구에서는 심리부검 면담을 통해 수집된 자살사망자 자료 중 사망 전 우울장애 진단을 받았던 210명의 자료를 분석에 활용하였다. 심리부검 면담을 통해 자살사망자의 우울장애 진단 여부 및 시기를 포함하여 생애 스트레스 사건, 진단 후 자살시도력, 사망 3개월 전 음주·수면·불안 문제의 증가, 가족의 정신건강 문제 등을 조사하였고, 빈도분석과 분산분석, 다중회귀분석을 진행하였다. 연구 결과, 연구대상자의 평균 우울장애 진단-자살사망기간은 53.42개월이었고, 전체의 3분의 1이 1년 이내에 자살사망한 것으로 나타났으며, 연구대상자의 인구통계학적 변인을 통제했을 때 사망 3개월 전 수면 문제와 불안 증상의 증가가 우울장애 진단-자살사망 기간에 유의한 영향을 미치는 것으로 나타났다. 분석 결과를 바탕으로 우울장애 조기 진단 및 치료적 개입, 우울장애 환자의 자살위험 평가 시 수면 문제 및 불안 증상 확인, 의료진 보수교육에 자살예방 교육 필수 포함, 직업 및 경제적 문제 예방을 위한 사회적 안전망 확충에 대해 제안하였다.

The purpose of this study is to identify the suicide risk factors associated with depression and propose measures to prevent suicide among individuals diagnosed with depression. We collected suicide death data through psychological autopsy interviews with bereaved families of 210 people who were diagnosed with depressive disorders before death. The interviews investigated stress events, suicide attempts after diagnosis, increased alcohol consumption, sleep disturbances, and anxiety symptoms in the three months preceding death, as well as family mental health problems and the time elapsed between suicide and diagnosis of depressive disorders. This study found that, on average, subjects experienced suicide 53.42 months after diagnosis, with a third of all suicides occurring within a year. Controlling for demographic variables, an increase in sleep problems and anxiety symptoms three months before death significantly affected the duration between depression diagnosis and suicide. Based on these findings, recommendations were made for early diagnosis and intervention for depression, assessment of sleep problems and anxiety symptoms in suicide risk evaluation for depressed patients, mandatory suicide prevention education for medical professionals, and expansion of social safety nets to prevent occupational and economic problems.
