본 논문에서는 국내에서 널리 사용되는 정밀주의집중력검사가 진단 도구로서 임상적으로 어떻게 적용 가능한지 알아보고자 하였다.

본 연구에는 2012 년 8 월부터 2014 년 5 월까지 서울아산병원 소아청소년 정신과 외래를 방문한 6~12 세 아동 114 명이 포함되었다. DSM-IV 진단기준 및 Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version(K-SADS-PL)에 의하여 79 명이 ADHD 로 진단되었고, 35명이 대조군으로 선정되었다. 연구에 참여한 아동은 모두 정밀주의집중력검사를 시행하였다.

본 연구에서 정밀주의집중력검사의 변수 별 적합도 검정을 시행하였을 때 시각검사의 누락오류, 오경보오류, 반응시간 표준편차 및 청각검사의 반응시간 표준편차의 분포가 ADHD 군과 대조군에서 달랐다. ADHD 군과 대조군 아동의 정밀주의집중력검사 결과를 진단기준에 따라 8 가지 변수 중 하나라도 1.5 표준편차 이상인 경우를 'ADHD'로 판단하였을 때, 본 연구 대상자에서의 민감도는 84.8%, 특이도가 45.7%로 측정되었다. 단계 선택법을 이용한 판별 분석에서 청각검사의 반응시간 표준편차, 시각검사의 오경보오류가 두 집단을 잘 구분해 주었다. 이를 통해 본 연구 대상에 판별식을 적용하였을 때, 정확도가 64.9%로 나타났다. ADHD 군 24 명과 대조군 15 명이 반복 검사를 시행하였고, 대조군에 비하여 ADHD 군이 급내 상관 계수가 낮은 경향을 보였다.

본 연구에서 정밀주의력검사의 청각검사의 반응시간 표준편차, 시각검사의 오경보오류와 같은 소척도들은 ADHD 군과 일반 아동을 구분하는데 유용한 변수로 나타나, 정밀주의력검사가 ADHD 의 신경심리학적 소견을 이해하는데 도움이 되는 것으로 나타났다. 그러나 ADHD 아동과 정상 아동은 그 분포에서 차이가 있기 때문에, 정상아동의 결과를 기반으로 한 T 점수 기준은 ADHD 를 진단함에 있어 제한이 있을 수 있겠다. 또한, 정밀주의력검사의 변수 가운데 1~2 가지만 이용하여 결과를 해석하거나 일회적인 검사로 ADHD 를 진단하는데 있어 주의를 기울여야 하겠다.
