2005년 BRCA1/2 돌연변이와 Poly (ADP-ribose) polymerase (PARP) 저해제 간의 합성치사(synthetic lethal) 관계가 밝혀 짐에 따라 현재 PARP 저해제는 BRCA1/2 돌연변이가 많은 난소암과 유방암, 전립선암 그리고 췌장암에서 항암치료 후 유지요법으로 사용되고 있는 약물이다. 하지만 실제 임상에서 모든 BRCA1/2 돌연변이 환자가 PARP 저해제에 반응성을 보이지는 않았으며, BRCA1/2 돌연변이가 없다 하더라도 반응성을 보이는 경우들이 있어 BRCA1/2 이외의 환자선별을 위한 생물학적 표지자에 대한 필요성이 제기되어왔다.
따라서 본 연구에서는 BRCA1/2 돌연변이가 없는 두가지 유방암 세포주에서 CRISPR-dCas9 interference 및 activation 시스템을 이용하여 유전자 발현을 조절함으로써 BRCA1/2 돌연변이 여부와 상관없이 약물반응성에 영향을 주는 조절자를 찾고자 하였다. 이전 연구들에서 밝혔듯이 DNA repair 과정에 관여하는 많은 유전자들이 PARP 저해제 반응성에 관여하는 것으로 확인되었으며, 특정 세포주와 상관없이 공통적인 약물 반응성 조절자로서 작용하는 것들은 PARP 저해제의 작용기전과 직접적으로 연관되는 DNA repair나 NAD⁺ metabolism과 관련된 것들이었다.
특이하게도 많은 미토콘드리아 단백질들이 그 발현이 저해될 경우, 세포내 산화적 인산화 과정이 감소되고 혐기적 해당과정에 대한 의존도가 높아지면서 PARP 저해제에 대한 반응성을 감소시키는 것으로 나타났다. 특히 해당과정에 대한 높은 의존도는 암의 주요 특징 중 하나인 Warburg effect 및 저산소 환경에서의 대사적 특징과도 연관되며, 이러한 대사적 특징은 PARP 효소의 활성을 높여 PARP 저해제의 작용을 방해하는 것으로 보인다.
본 연구는 기존에 제시된 DNA repair 관련 유전자 외에도 세포내 에너지대사에 따라 PARP 저해제에 대한 반응성이 달라짐을 BRCA1/2 돌연변이 여부와 상관없이 유전체수준에서 밝혀냈다. 또한 현재 임상에서 활용하고자 하는 MTOR 저해제에 대한 재평가의 필요성을 제시하였으며, 항암치료후 증가된 ABCC2가 olaparib에 대한 반응성을 감소시킬 수 있음을 보여주었다.*표시는 필수 입력사항입니다.
