초록보기

 전세계 인구는 전례 없는 고령화의 흐름을 겪고 있습니다. 2024년 기준, 65세 이상의 인구는 전체의 약 10%를 차지하고 있으며, 2050년에는 이 비율이 16%를 초과할 것으로 전망됩니다. 이러한 고령화는 평균 기대수명의 급격한 증가와 함께 나타나고 있으며, 1990년 64세였던 기대수명은 2019년 72.8세로 증가했으며, 2050년에는 약 77.2세에 이를 것으로 예측됩니다. 이와 같은 변화는 의료 및 사회경제적 발전의 결과이지만, 동시에 노화 연관 질환의 부담 증가를 시사합니다.

생물학적 노화는 생리적 기능과 조직 재생 능력의 점진적인 저하로 특징지어지며, 이는 분자적 및세포 수준의 복합적인 변화에 의해 주도됩니다. 노화의 주요 특징으로는 텔로미어 단축, 유전체 불안정성, 후생유전학적 변화, 영양 감지 이상, 미토콘드리아 기능 저하, 그리고 만성 염증(inflammaging) 등이 있습니다. 이러한 변화들은 조직 항상성을 손상시키고, 심혈관 질환, 신경퇴행성 질환, 암, 만성 신장 질환 등 다양한 비감염성 질환(NCDs)의 발생 위험을 증가시킵니다.

본 논문에서는 노화와 밀접한 관련이 있는 두 가지 주요 질환, 즉 공격적인 골수성 백혈병과 신장 섬유화 질환을 중점적으로 다룹니다. 두 질환 모두 노화 관련 분자적 변화에 크게 영향을 받지만, 각각의 병태생리적 환경에서 노화가 미치는 영향은 상이하게 나타납니다. 공격적인 골수성 백혈병에서는 DNA 손상과 산화적 스트레스의 축적이 초기에는 백혈병 전환을 유도하지만, 과도한 스트레스는 오히려 암세포의 사멸을 유도할 수 있습니다. 반면, 신장 질환에서는 지속적인 DNA 손상과 산화 스트레스가세포 노화를 유발하여 섬유화와 조직 기능 저하를 촉진합니다.

이러한 상반된 결과는 노화 조직 내 스트레스 적응 기전이 질환 맥락에 따라 다르게 작용함을 시사합니다. 다양한 병리적 환경에서 노화된세포가 대사적, 유전독성 및 산화적 스트레스에 어떻게 반응하는지를 이해하는 것은, 노화에 적합한 맞춤형 치료 전략 개발에 있어 매우 중요합니다. 본 연구는 이러한 메커니즘을 규명하고, 노화 관련 현액암과 신장 섬유화 질환에 대한 치료적 접근을 모색하는 데 목적을 두고 있습니다.
