연구배경

우울증의 중요성에 비해 우리나라에서는 일반 인구를 대상으로 표준화된 측정도구, 특히 Beck Depression Inventory(BDI)를 사용한 연구는 매우 드물다. 본 연구에서는 성인 연령군 전체를 대상으로 강원도 춘천시 사북면 주민의 우울 증상의 유병률을 구하고 관련요인들을 분석하여 농촌 지역의 우울증상 실태를 조사하고자 하였다.

방 법

직접 면접 설문 조사방법을 통해 자료를 수집하였다. 총 179명이 BDI의 모든 항목 및 사회인구학적 질문에 답변하였다. 다변량 로지스틱 회귀분석을 이용하여 우울 증상 및 관련 요인을 분석하였다.

결 과

연구 대상의 우울 증상 유병률은 남자가 19.4%, 여자가 21.5%였고, 직접법으로 연령 보정한 후의 우울 증상 유병률은 각각 27.8%, 21.8%였다. 우울 증상과의 관련 요인을 분석하기 위해 다변량 로지스틱 회귀분석을 시행한 결과 우울 증상과 관련이 있는 요인은 결혼 상태(오즈비 = 3.62, 95% 신뢰구간 1.17-11.24)와 사회적 지지(오즈비 = 5.80, 95% 신뢰구간 1.51-22.20), 스트레스 정도(오즈비 = 4.39, 95% 신뢰구간 1.57-12.24)로 나타났다.

결 론

우울증상에 대한 지역 정신보건 사업은 사회 소외 계층과 스트레스 위험군, 배우자가 없는 군을 우선순위 대상 집단으로 하여 서비스를 제공하는 것이 바람직할 것이다.

Background

Despite the fact that depression has become a serious social problem, there is little research on the prevalence of depressive symptoms in the general population, using the standardized screening instruments, especially Beck Depression Inventory (BDI) in Korea. The aims of this study were to estimate the prevalence of depressive symptoms and to analyze their association with risk factors among adults in a rural community in Korea.

Methods

A survey-based, cross-sectional descriptive study was carried out. A total of 179 participants completed the BDI with a sociodemographic questionnaire. Multiple logistic regression analysis was performed for comparisons.

Results

The prevalence of depressive symptoms in men was 19.4% and that in women was 21.5%. After adjusting for age, the prevalence was 27.8%, 21.8% respectively. Depressive symptoms was associated with marital dissolution (separated/divorced/widowed) [odds ratio (OR) = 3.62 , 95% confidence interval (CI) = 1.17-11.24], poor perceived social support (OR = 5.80, 95% CI = 1.51-22.20) and high self-assessed level of stress (OR = 4.39, 95% CI = 1.57-12.24).

Conclusions

Mental health program of local health authorities should focus on underprivileged people, especially for those who are alienated, vulnerable to stress and experienced marital breakdown.
