호지킨림프종은 다른 혈액암에 비하여 생존율이 높은 질환이나 일부 환자는 질환이 완치되지 못하므로, 생존율 향상을 위한 노력이 필요하다. 항암치료 후 재발한 환자에서 구제항암화학요법 이후 반응이 있는 경우 고용량항암화학요법 및 자가조혈모세포이식을 시행하는 것이 현재의 표준치료이다. 일부 고용량항암화학치료가 불가능한 환자에서는 통상 용량의 항암치료 혹은 국소재발이면서 이전에 항암화학치료만 시행받은 경우 2차 방사선 단독치료를 고려할 수 있다. 동종조혈모세포이식은 이식편대항림프종 효과를 얻어 완치 확률을 높일 수 있으나 지속적인 재발과 치료 관련 독성이 문제가 되고 있어 선택적으로 임상시험으로 시행하는 것이 권유된다. 그리고 최근 많은 신약 들이 개발되어 활발한 임상연구가 진행되고 있으며, 면역조절 및 세포면역치료는 기존의 항암화학치료나 방사선 치료로 효과가 없었던 일부 환자에서 완전관해를 포함한 치료 반응 유도가 보고되고 있어 치료법 강화를 통한 치료 성적 향상을 기대할 수 있겠다.
Hodgkin lymphoma has been a curable disease, however some patients have relapsed or refractory disease. The standard treatment of these patients is salvage chemotherapy followed by high dose therapy and autologous stem cell transplant (HDT with ASCT) in patients who are chemotherapy-sensitive. Some portion of the patients not eligible for HDT with ASCT because of old age, chemo-refractory and major comorbidities, can be offered alternative approaches such as radiotherapy or conventional chemotherapy. Allogeneic stem cell transplantation could give some benefit in terms of a graft-versus-lymphoma effect. However due to significant toxicity, this approach should be recommended in the context of clinical trials. Biologic markers are served as prognostic markers as well as therapeutic targets. New therapies including novel agents and immune cell therapies are currently being developed. Immune modulatory and immune cell therapies can be effective even in chemo-refractory patients. Efforts should be focused on progression to overcome tumor evasion mechanism and development of better treatment strategies. (Korean J Med 2011;81:569-574)
