목적: 뇌동맥류 파열에 의한 지주막하출혈 후 발생하는 뇌혈관연축은 환자의 이환율과 사망률에 영향을 미치는 주요한 인자 중의 하나이다. 뇌혈관연축 환자에서 동맥 내 니모디핀 주입술의 치료효과를 알아보고자 하였다.

대상과 방법: 2005년 2월부터 2011년 4월까지 동맥류성 지주막하출혈로 수술이나 코일색전술 후 뇌혈관연축이 발생하여 동맥 내 니모디핀 주입술로 치료한 50명의 환자를 대상으로 후향적으로 분석하였다. 뇌혈관조영술을 시행하여 뇌혈관연축이 있는 경우 카테터를 통하여 니모디핀을 분당 0.1 mg의 속도로, 한 혈관당 2~3 mg을 주입하였다. 시술 직후의 혈관조영술에서 혈관연축의 호전 정도를 평가하였다. 또한 퇴원시 환자의 임상결과를 Glasgow Outcome Scale (GOS)로 평가하였으며, grade 5와 4를 만족스러운 결과를 보인 것으로 하였다.

결과: 전체 50명의 환자에서 모두 117회(1~7; 평균, 2.3)의 니모디핀 주입술을 시행하였다. 시술 직후 시행한 혈관조영술에서 혈관연축의 호전은 113예(96.6%)에서 관찰되었으며, 시술로 인해 신경학적 후유증을 동반하는 합병증은 보이지 않았다. 퇴원 시점의 GOS 평가에서 38명(76%)에서 만족스러운 임상결과를 보였다.

결론: 동맥 내 니모디핀 주입술은 동맥류성 지주막하출혈에 의한 뇌혈관연축에 효과적이고 안전한 치료법으로 생각된다. 그러나 2회 이상 반복적으로 시행하여야 하는 문제점을 가지고 있다.

Purpose: Symptomatic cerebral vasospasm following an aneurysmal subarachnoid hemorrhage (SAH) is one of the major factors which cause morbidity and mortality of patients. The purpose of this study was to evaluate the effectiveness of intra-arterial nimodipine (IAN) infusion therapy in patients with symptomatic cerebral vasospasm.

Materials and Methods: Between February 2005 and April 2011, fifty patients with symptomatic cerebral vasospasm following an aneurysmal SAH were treated with IAN infusion. After selective arterial catheterization, nimodipine was infused at a rate of 0.1 mg/min and a total of 2-3 mg per vessel was infused. We retrospectively reviewed the immediate angiographic results and clinical outcome at discharge. A grade of 5 and 4 in the Glasgow Outcome Scale (GOS) were considered favorable outcomes.

Results: In 50 patients, 117 procedures of IAN infusion (1-7; mean, 2.3) were done. After the treatment, immediate angiographic improvement was achieved in 113 (96.6%) of 117 procedures. No major complications occurred. At discharge, 38 (76%) patients showed a favorable clinical outcome in the GOS.

Conclusion: IAN infusion therapy is safe and effective for the treatment of cerebral vasospasm following an aneurysmal SAH. However, the limitation is that repeated treatment is needed.
