국내 항생제 사용관리 프로그램의 최적화 방향에 대한 제언 : 미국 및 일본의 사례를 참고하여 = Suggestions for optimizing the antimicrobial stewardship program in South Korea : with reference to ASP initiatives in the US and Japan
BACKGROUND Antimicrobial Stewardship Program(ASP) is system for managing the usage of antibiotics in hospital which is essential to optimize patient care and reduce antimicrobial resistance. Since MDRs(Multidrug-Resistant Organisms) become emerging problem and high antimicrobial resistance appeared, many health care professionals emphasize the importance of implementing ASP in Korea. For successful antimicrobial management, ASP with multidisciplinary team with professionals such as physicians, nurse and pharmacists is needed. METHODS Qualitative research methods based on articles, governmental reports and guidelines were employed for comparing current state of operating ASP. Some data offered by HIRA and KOSIS were used for producing statistics. RESULTS In South Korea, ASP is only implemented in some of tertiary hospitals yet the usage of antibiotics are even high in the primary hospitals. To optimize ASP for effective implementation, ASP guidelines for primary and secondary hospitals is needed. Also antibiotic use for outpatient should be considered for ASP since the high prescription amount of antibiotics is seen. To suggest improvement plan for domestic ASP, this article refer to guidelines and system in the U.S. and Japan. CONCLUSIONS Eventually, sufficient reward system and nurturing professionals in the field is the most important elements for implementing ASP in South Korea. Also medical environment and hospital size should be appropriately considered for practical implement in ASP.
