Objective: : In this study, we aimed to compare how resistance exercise using elastic bands and focused extracorporeal shockwavetherapy affect pain, vastus medialis and vastus lateralis muscle activities, and knee function in patients with patellofemoral painsyndrome (PFPS).

Design: Two group pre-post test designMethods: In this study, we involved 39 patients with patellofemoral pain syndrome, dividing them into following groups: elasticband-using resistance exercise and an extracorporeal shock wave therapy group. We administered the interventions twice daily for30 min over six weeks. Outcome measures included the Kujala patellofemoral score, visual analog scale, Victorian Institute ofSports Assessment patellar questionnaire, and vastus medialis, and lateralismuscle activities, as – well as their ratio.

Results: Resistance exercises using elastic bands and extracorporeal shock wave therapy significantly improved pain, kneefunction, and muscle activity in patients with patellofemoral pain syndrome.Resistance exercise demonstrated a significantadvantage over shockwave therapy in terms of pain reduction (p < 0.05). We observed no significant differences between thegroups in the Kujala patellofemoral score, Victorian Institute of Sports Assessment-patellar questionnaire, muscle activity, andvastus medialis to vastus lateralis ratio.

Conclusions: The results of this study suggest that while both treatments were effective, resistance exercises using elastic bandsmight be more beneficial for pain management in patients with patellofemoral pain syndrome.
