Organoids, three-dimensional structures developed from pluripotent or adult stem cells, haveemerged as a transformative tool for modelling human tissues in vitro. These models more accuratelymimic key features of native organs than traditional cell cultures or animal models,better aiding in the understanding of human-specific physiology and disease mechanisms. Gastrointestinalorganoids have demonstrated considerable utility in drug discovery and screening,personalized medicine, and research on organ development, carcinogenesis, and host–microbiotainteractions. Their relevance in the domains of drug screening and toxicological testingcontinues to expand, especially as efforts to reduce interspecies variability and euthanasia inpreclinical research grow. This review underscores significant milestones in the developmentand application of gastrointestinal organoids, particularly those derived from colon and colorectalcancer tissues, for drug testing and toxicity assessment. Colorectal cancer organoids allowfor the capture of patient-specific heterogeneity and have facilitated personalized approaches totherapeutic assessment. Despite persistent challenges, including limited immune and stromalintegration and technical inconsistencies across laboratories, recent advancements, such as coculturetechniques, organ-on-a-chip systems, and bioengineered matrices, have broadened theapplicability of organoids. Building on these technological advancements, organoid technologyis expected to play an increasingly significant role in advancing precision medicine for colorectalcancer and in developing human-relevant toxicity assessment strategies.
