본 연구의 목적은 노인의 사전연명의료의향서 작성 관련 요인을 건강상태, 죽음에 대한 태도, 죽음 준비 측면에서 파악하는 것이다. 이를 위해 2023년 노인실태조사 원시자료 중 일부를 활용하여 이차 자료분석을 수행하였다. 원시자료는 2023년 9월부터 11월까지 수집되었고, 이 중 설문에 직접 응답한 9,951명을 최종 연구대상자에 포함하였다. 자료분석은 IBM SPSS/WIN 23.0 프로그램의 기술통계분석, 교차분석, 독립 표본 t 검증, 가중치를 적용한 이항 로지스틱 회귀분석을 이용하였다. 그 결과, 사전연명의료의향서 작성의 주요 관련 요인은 복합만성질환(OR=1.62, CI: 1.385-1.903), 우울 위험(OR=1.39, CI: 1.183-1.637), 스스로 정리한 후 죽음(OR=1.34, CI: 1.193-1.505), 죽음 준비 교육 수강(OR=1.93, CI: 1.412-2.632), 유서 작성(OR=2.19, CI: 1.626-2.941), 장기기증 서약서 작성(OR=4.17, CI: 3.222-5.396) 등이었다. 이러한 결과는 노인의 사전연명의료의향서 작성과 건강 상태, 품위 있는 죽음, 죽음 준비 상태와의 관련성을 보여준다. 특히, 다른 요인들이 교차비가 작거나 통계적으로 유의하지 않는 것에 비해, 죽음 준비 교육을 받거나, 장기기증 서약서나 유서를 작성했을 때 사전연명의료의향서 작성이 2배 이상 높았다. 이 세 가지 요인은 주로 노인의 인지기능에 손상이 없을 때 수행된다. 그러므로 사전연명의료의향서에 대한 설명도 노인의 인지기능이 온전하고 건강상태가 좋을 때 수행하여 연명의료에 대해 스스로 선택할 기회를 제공해야 할 것이다.

The purpose of this study was to identify factors associated with advance directive completion among older adults, focusing on health status, attitudes toward death, and preparation for death. A total of 9,951 samples from the 2023 Survey of the Elderly were subjected to secondary data analysis. Data were analyzed using descriptive statistics, chi-square analysis, the independent sample t-test, and weighted binary logistic regression analysis in IBM SPSS/WIN 23.0. Key findings revealed that completing an advance directive was significantly associated with multimorbidity (OR=1.62, 95% CI: 1.385-1.903), depression risk (OR=1.39, 95% CI: 1.183-1.637), putting one's affairs in order before death (OR=1.34, 95% CI: 1.193-1.505), participating in death preparation education (OR=1.93, 95% CI: 1.412-2.632), writing a will (OR=2.19, 95% CI: 1.626-4.41), and pledging organ donation (OR=4.17, 95% CI: 3.222-5.396). These results show that advance directive completion by older adults is strongly related to death preparation education, organ donation pledge, or writing a will. On the other hand, the increase in odds for other factors was either negligible or statistically insignificant. Since these three factors are primarily considered when cognitive function and health are intact, information about advance directives should be provided at that time.
