본 연구는 성범죄자의 재범을 조기에 탐지·예방하기 위해 전자감독 체계에 생리·심리 모니터링 기술을 통합하는 방안의 가능성을 검토하였다. 2020~2025년에 발표된 국내외 연구를 문헌고찰하여 생체신호 기반 모니터링의 기술적 구현 가능성, 심박변이도(HRV)와 호르몬 수준 간 상관관계, 광용적맥파(PPG) 등 생체신호를 활용한 호르몬 관련 변화의 간접 추정 가능성, 성적 충동성·재범 위험 및 호르몬 변동 간 관계를 분석하였다. 고찰 결과 HRV 등 자율신경계 지표는 스트레스 및 충동성과 연관된 생리적 변화를 포착하며, 호르몬 변동과도 유의미하게 관련되는 경향이 확인되었다. 특히 테스토스테론은 성범죄자의 충동성과 공격성에 영향을 미치는 핵심 요인으로 제시되었고, 테스토스테론을 약리적으로 낮추는 치료는 재범 위험을 유의미하게 감소시키는 반면 치료 중단 시 위험이 재상승하는 경향이 보고되었다. 또한 세로토닌은 충동 조절에 관여하는 요인으로 나타났으며, 선택적 세로토닌 재흡수 억제제(SSRI) 치료가 성적 충동을 억제하고 재범률을 낮출 수 있다는 보고도 확인되었다. 이러한 결과는 생체신호 분석을 통한 재범 위험 모니터링이 이론적으로 가능함을 시사하며, 향후 전자감독 전략의 고도화 및 맞춤형 위험관리 체계 설계에 기초 자료를 제공한다.

This study reviews domestic and international literature published between 2020 and 2025 to assess the feasibility of integrating physiological and psychological monitoring technologies into electronic ankle monitoring systems for the early detection and prevention of sexual-offense recidivism. The review focused on the technical feasibility of biometric signal monitoring, evidence linking heart rate variability (HRV) to hormonal dynamics, the potential for indirectly inferring hormone-related changes using photoplethysmography (PPG) and related autonomic indicators, and relationships among sexual impulsivity, recidivism risk, and hormone-associated changes. The findings suggest that biometric markers—particularly HRV—are meaningfully associated with hormonal fluctuations and can capture physiological changes linked to stress and impulsive states. Testosterone emerged as a key factor associated with impulsivity and aggression among sexual offenders. Studies consistently reported that pharmacological suppression of testosterone was associated with reduced recidivism risk, whereas discontinuation of treatment was followed by renewed or increased risk. Serotonin-related mechanisms also appeared relevant to impulse control, and selective serotonin reuptake inhibitors (SSRIs) were reported to attenuate sexual urges and contribute to lower recidivism in some contexts. Overall, the reviewed evidence indicates that biometrics-based monitoring of recidivism risk is theoretically plausible and may inform the design of next-generation electronic monitoring strategies.
