본 연구는 정신장애인 가족지원을 다룬 국내 연구를 체계적으로 분석하여 정신장애인 가족을 위한 지원이 어떠한 내용과 유형으로 이루어졌으며, 지원과정에서 가족의 역할과 참여 수준은 어떠한지 살펴보고자 수행되었다. 분석 대상 논문 선정 기준에 따른 검색과 추출 과정을 통해 총 28편의 논문이 선정되었고, 이 논문을 출간 연도, 연구설계, 진단군, 지원 유형, 참여 가족 유형, 가족 참여 수준을 중심으로 분석하였다. 분석 결과, 조현병 가족을 위한 연구가 가장 많았고, 교육과 상담⋅치료가 지원 유형 중 가장 높은 비중을 차지하였다. 가족 참여 수준은 가족이 정신장애인의 회복을 돕는 ‘매개적 참여자’로서의 참여가 가장 많았고, 직접적 참여와 매개적 참여가 공존하는 혼합형도 높은 비중을 보였다. 대부분의 연구가 가족 구성원 1인의 참여를 기반으로 이루어져 가족 전체의 역동이나 상호작용 변화를 다루는 지원은 제한적이었다. 본 연구는 정신장애인 가족지원 국내 연구의 동향을 조망하고 가족지원 과정에서 가족의 참여 수준을 구체적으로 고찰함으로써 국내 가족지원 연구가 전문가 중심 개입에서 가족 중심 접근으로 이행하고 있음을 확인하고 향후 가족 중심⋅관계 중심 지원 모델 확장의 필요성을 제시했다는 의의가 있다.

This study conducted a systematic review of domestic research on support for families of individuals with mental disorders to examine the contents and types of support provided, as well as the roles and levels of family participation in the support process. Through a systematic search and extraction process based on inclusion criteria, a total of 28 studies were selected and analyzed according to publication year, research design, diagnostic group, support type, participating family type, and level of family participation. The analysis revealed several key findings. First, research targeting families of individuals with schizophrenia was the most prevalent, and “education” and “counseling/therapy” accounted for the largest proportion of support types. Second, regarding the level of family participation, the most frequent role was that of a “mediated participant” assisting in the recovery of the individual with a mental disorder; the “mixed type,” where direct and mediated participation coexist, also showed a high frequency. Third, most studies were based on the participation of a single family member, indicating a limitation in support programs that address the dynamics or interactional changes of the entire family unit. This study is significant in that it identifies a paradigm shift from professional-centered intervention to a family-centered approach by outlining research trends and examining the specific levels of family involvement. Based on these findings, this study suggests the necessity of expanding family-centered and relationship-oriented support models in the future.
