 본 연구는 로이의 적응 모델을 적용하여 동종 조혈모세포이식 환자의 이식 후 초기 면역력 재구축 기간의 삶의 질 변화 양상 및 이에 영향을 미치는 요인을 파악하기 위한 전향적, 종단적 서술연구이다. 연구 대상은 J도에 위치한 C 대학교 병원에서 동종 조혈모세포이식을 받은 환자로 이식 전, 이식 후 30일, 100일에 설문조사를 실시하였다. 자료수집 기간은 2012년 11월부터 2014년 3월까지였고 44명(91.7%)의 환자가 연구에 참여하였다. 주요한 측정 변수는 대처 유형, 증상 중증도, 우울, 사회적 의존성, 외로움 및 삶의 질이었으며, 각 변수는 스트레스 대처 설문지, M.D. Anderson 증상 조사지, Beck 우울 측정도구, 사회적 의존성 측정도구, UCLA 외로움 척도, 삶의 만족도 척도로 측정하였다. 수집된 자료는 Window SPSS program (Version 21.0/PC)과 HLM 7.01을 이용하여 분석하였다. 시간 경과에 따른 삶의 질 변화 양상 및 영향 요인을 확인하기 위하여 다층 모형으로 분석하였다.

연구 결과를 요약하면 다음과 같다.

동종 조혈모세포이식 환자의 이식 후 초기 면역력 재구축 시기의 삶의 질은 감소하며, 저강도 전처치군은 이식 전과 이식 후 100일에 정서중심대처를 유의하게 많이 사용하는 것으로 나타났다. 또한 시간 경과에 따라 증상 중증도와 외로움 정도는 전처치 유형에 따라 다른 변화 양상을 보였으며, 증상 중증도와 사회적 의존성 정도는 이식 후 30일에 유의하게 높아지는 것으로 나타났다. 이식 전과 이식 후 100일에 대상자가 문제중심대처를 이용할수록 외로움의 정도는 감소하였으며, 증상 중증도와 우울 및 외로움은 세 측정 시점 모두에서 유의한 상관관계가 있었다. 본 연구 대상자들의 적응 수준은 보상수준이었으며, 전처치 유형은 이식 전의 삶의 질과 평균 변화율에 유의한 영향을 미치는 요인이었다. 저강도 전처치군은 이식 전에 삶의 질이 고강도 전처치군에 비해 유의하게 높았으나 시간 경과에 따라 낮아지는 경향을 보인 반면 고강도 전처치군은 높아지는 경향을 나타냈다. 증상 중증도, 우울, 사회적 의존성 및 외로움은 삶의 질에 부적 효과를 나타냈으며 특히 증상 중증도와 우울 정도가 유의하게 영향을 미치는 요인이었다. 즉 증상 중증도와 우울 정도가 높을수록 삶의 질이 낮아지는 것으로 나타났다.

이상의 연구 결과를 바탕으로 동종 조혈모세포이식 후 초기 면역력 재구축시기에 대상자들의 삶의 질을 향상 시킬 수 있는 중재 프로그램 개발이 필요하다. 특히 대처 능력 향상 및 증상 중증도와 우울을 조절하기 위한 중재 요법이 다학제적 협력으로 이식 전 시기부터 꾸준히 이루어져야 할 것으로 여겨진다.
