Purpose:The aim of this study was to investigate the epidemiologic status of Kawasaki disease (KD) in infants ≤6 months of age.

Methods:For the epidemiologic study of KD in Korea, data from 22,674 KD patients were collected from 1997 to 2005 on a 3-year basis by a retrospective survey. From this survey, data of 1,739 KD patients ≤6 months of age were analyzed and compared with those of KD patients >6 months of age.

Results:A total of 1,739 patients ≤6 months of age, including 1,088 males and 651 females, represented 7.7% of total KD patients. These subjects included 22 patients aged <1 month, 171 patients aged 1-2 months, 304 patients aged 2-3 months, 407 patients aged 3-4 months, 372 patients aged 4-5 months, and 463 patients aged 5-6 months. Their mean age of onset was 4.3 months, and the male-to-female ratio was 1.67:1. Incidences of coronary arterial (CA) abnormalities (21.0% versus 18.7%) and CA aneurysms (4.7% versus 3.1%) detected by echocardiography showed differences between patients with KD younger and older than 6 months, respectively.

Conclusion:Comparison of data from KD patients >6 months old with data from 1,739 KD patients ≤6 months old showed significantly higher incidences of CA abnormalities and CA aneurysms in the younger patients. (Korean J Pediatr 2008;51: 1320-1323)

목 적 : 치명적인 관상동맥 합병증을 일으킬 수 있는 가와사끼병에 있어, 6개월 이하의 소아에서의 역학적 특성을 알아보기 위하여 본 연구를 시행하였다.

방 법 : 매 3년 단위로 시행되는 가와사끼병의 국내 역학조사에서, 1997-2005년까지 9년간의 전체 환아 22,674예 중 발병 연령이 6개월 이하인 환아 1,739예를 대상으로 하였다. 조사문항은 1) 나이, 성별 및 발병연월일, 2) 가족증례 및 재발여부, 3) 심초음파 시행여부 및 소견, 4) 관상동맥 조영술의 시행여부 및 소견, 5) 심근경색과 사망 등의 합병증, 6) 대상병원의 신생아실을 제외한 소아 병동에 입원한 전체 환아 중에서 가와사끼병의 환아가 차지하는 비율에 대한 것이었으며, 이를 6개월이 넘는 환아군과 비교분석을 시행하였다.

결 과 : 전체 환아 22,674예 중 7.7%인 1,739예가 6개월 이하로, 0-1개월이 22명, 1-2개월은 171명, 2-3개월은 304명, 3-4개월은 407명, 4-5개월은 372명, 5-6개월은 463명의 분포를 보였다. 평균 연령은 4.3개월 이었으며, 남녀비는 1.67이었다. 심에코검사상 관상동맥류와 관상동맥 이상소견 빈도에 있어 각각 4.7%와 21.0%를 보였는데, 이는 6개월이 넘는 환아에서의 3.1%와 18.7% 보다 높은 수치였다.

결 론 : 발병 연령이 6개월이 넘는 환아군과 비교해본 결과, 6개월 이하인 환아군의 관상동맥 이상소견과 관상동맥류의 발생에 있어서 통계적으로 의미 있게 높은 빈도를 나타내었다.
