연구배경: 담낭용종은 건강 검진에서 초음파 검사가 보편화됨에 따라 종종 발견된다. 일부 보고에서 체질량지수와 담낭용종의 연관성에 대해서 보고하였다. 최근 비만의 유병률은 증가하고 있는 상황이다. 그래서 우리는 한국 건강관리 협회의 자료를 토대로 한국에서 담낭용종의 유병률과 연도에 따른 유병률의 변화 유무를 조사하였다.

방법: 한국건강관리협회에서 2003년 1월2일부터 2007년 8월 3일까지 복부 초음파 검사를 받은 수검자를 후향적 단면 연구하였다. 20세 미만이거나 동양인이 아닌 사람과 담낭절제술을 받은 경우는 조사에서 배제하였다. 유병률 조사를 위한 20세 이상 성인의 대상자는 137,135명이었다. 남자 성인은 69,493명이었고, 여자 성인은 67,642명이었다.

결과: 전체 대상자 137,135명에서 담낭용종이 발견된 수검자는 5,278명이었고, 남자는 3,163명, 여자는 2,115명이었다. 통계청의 2005년 인구조사 자료에 따른 연령별 보정으로 전체 인구의 유병률은 인구 10만 명 당 3,656명(3.7%)이고, 남자는 인구 10만 명 당 4,403명(4.4%), 여자는 인구 10만 명 당 2,943명(2.9%)이었다. 담낭용종의 유병률은 최근 5년 동안 증가하는 경향을 보였다.

결론: 한국에서 담낭용종의 유병률은 기존의 보고보다 높게 나타났다. 남자의 유병률이 여자보다 높았다. 최근 5년간 담낭용종의 유병률은 증가하고 있으며 담낭용종에 더 많은 관심을 기울일 필요가 있다.

Background: Incidentally discovered gallbladder polyp in a health check-up has been increasing in numbers due to the extended use of ultrasonography. Past research has reported on the relationship between body mass index and gallbladder (GB) polyp. As there has been a recent rise in the prevalence of obesity, this research attempted to study GB polyp prevalence and the trend according to time elapse in Korea.

Methods: Retrospective cross sectional research was carried out from the data of Korean Association of Health Promotion, between January 2, 2003 to August 3, 2007 in the subjects who received abdominal ultrasonography. The subjectrs excluded were those below 20 years of age, non-Asian and those who have received cholecystectomy. The number of eligible subjects were 137,135, where 69,493 were males and 67,642 were females.

Results: Out of 137,135 subjects, the number of subjects with GB polyp were 5,278, where 3,163 were males and 2,115 were females. According to 2005 census, the age adjusted prevalence was 3.7% (3,656 per 100,000). The prevalence for males was 4,403 per 100,000 (4.4%) and for females 2,943 per 100,000 (2.9%). The prevalence has been increasing for the past five years.

Conclusion: The prevalence of GB polyp in Korea was found to be higher than the pre-existing reports and the prevalence was higher in males than females. The prevalence is on the increase and there is a need to be more attentive to this issue in the future.
