목적: 본 연구는 제5 중수골 경부 골절의 치료에 있어서 K-강선을 이용한 경피적 횡고정술의 방사선학적 및 임상적 결과를 알아보고자 한다.

대상 및 방법: 2007년 1월부터 2010년 9월까지 제5 중수골 경부 골절로 수술을 시행한 18예를 대상으로 하였다. 수술적 방법은 K-강선을 이용한 경피적 횡고정술을 이용하였다. 방사선학적 평가는 수술 전, 수술 후, 최종 각형성 각도를 비교하고 SPSS 통계를 이용하여 분석하였다. 임상적 평가는 Disabilities of Arm, Shoulder and Hand (DASH) 점수를 이용하여 분석하였다.

결과: 전 환자에서 골유합이 관찰되었으며, 사면 방사선사진상 술 전 각형성 평균 50.69o에서 술 후 평균 11.68o로 교정되었다. 수술 후와 최종 추시 간 평균 0.14o 각형성이 증가하여 평균 11.81o가 관찰되었으나 통계상 유의하지 않았다. 임상적으로는 DASH 점수가 평균 1.030을 보였으며 어떤 합병증도 관찰되지 않았다.

결론: K-강선을 이용한 경피적 횡고정술은 수술 술기가 단순하고, 합병증이 낮은 방법으로 제5 중수골 경부 골절 치료 방법을 고려할 때 좋은 치료 방법의 하나로 판단된다.

Purpose: To evaluate the radiologic and clinical results of percutaneous transverse fixation with K-wires for 5th metacarpal neck fracture.

Materials and Methods: Between January 2007 and September 2010, 18 patients with a 5th metacarpal neck fracture, who underwent operative treatment, were included in this study. The surgical method was percutaneous transverse fixation using K-wires. We evaluated fracture angulation in oblique radiographs preoperatively, postoperatively, and at final follow-up, and used SPSS to perform statistical analysis. We also performed clinical evaluation using the Disabilities of the Arm, Shoulder and Hand (DASH) score.

Results: All of the 18 cases were completely united, and in the oblique radiographs, the angulation was corrected from 50.69o to 11.68o. The average difference between postoperative and final follow-up angulations was 0.14o, which was statistically insignificant. Clinically, the DASH score was 1.030 and no complications were observed.

Conclusion: Percutaneous transverse fixation using K-wires could be one of the best ways to treat a 5th metacarpal neck fracture because of its simple method and low rate of complications.
